低密度脂蛋白颗粒与代谢性血管损伤

摘要 代谢性血管损伤主要是由代谢紊乱导致的血管结构和功能异常。代谢紊乱又常伴有血脂水平异常导致代谢性血管损伤。长期以来临床血脂检测主要以低密度脂蛋白胆固醇(low-density lipoprotein cholesterol, LDL-C)为主,但大量临床数据表明,即使LDL-C水平控制正常,仍有部分患者具有心血管疾病的残余风险,低密度脂蛋白颗粒(low-density lipoprotein particle, LDL-P)在LDL-C和LDL-P水平不一致的个体中更能准确反映血脂相关风险。因此,LDL-P在代谢性血管损伤中可能是更为敏感的预测因子。本文主要介绍LDL-P的定义、结构、检测方法,及其在代谢性血管损伤中的作用机制以及降低LDL-P的治疗策略。 Metabolic vascular injury primarily manifests as structural and functional abnormalities of the blood vessels due to metabolic disorders. Metabolic disorders are often accompanied by abnormal lipid levels leading to metabolic vascular injury. For a long time, clinical lipid testing has been mainly based on low-density lipoprotein cholesterol(LDL-C) as the monitoring index. However, a large amount of clinical data has shown that even if LDL-C levels are normalized, some patients are still at residual risk for cardiovascular diseases, and low-density lipoprotein particles(LDL-P) are a more accurate reflection of the lipid-related risk in individuals with discordant LDL-C and LDL-P levels. Therefore, LDL-P may be a more sensitive predictor of metabolic vascular injury. This article mainly introduces the definition, structure, and detection methods of LDL-P, its mechanism in metabolic vascular injury and therapeutic strategies for reducing LDL-P.
