摘要
目的探讨浮桥技术矫正轻、中度上睑下垂的疗效。方法回顾性纳入2021年6月至2023年12月徐州医科大学附属徐州市立医院采用浮桥技术矫正轻、中度上睑下垂患者132例(169只眼),男49例,女83例,年龄18~35(27.4±7.5)岁,包括轻度上睑下垂28例,中度上睑下垂104例。术中将上睑提肌腱膜-米勒氏肌复合瓣以褥式缝合锚定至睑板,并行3层固定,为上睑提升提供力量传导。所有患者术后随访时间为10(6,12)个月。分别于术前、术后随访时测量上睑缘角膜映光距离(MRD1),评估美学效果、并发症发生率、术后复发率。结果术前MRD1为(1.3±0.3)mm,术后为(3.6±0.4)mm,差异有统计学意义(P<0.001)。132例患者中,120例(90.9%)术后效果评级为良好,8例(6.1%)评级为一般,4例(3.0%)评级为差。169只眼中,13只(7.7%)表现为矫正不足,8只(4.7%)表现为矫正过度,11只(6.5%)复发,未出现暴露性角膜炎。结论浮桥技术矫正轻、中度上睑下垂安全、有效,术后上睑运动功能恢复较好,外观效果佳。
ObjectiveTo explore the clinical effect of floating bridge technique in the correction of mild to moderate ptosis of upper eyelid.MethodsThis retrospective study included 132 patients(169 eyes)with mild to moderate ptosis who underwent ptosis correction using the floating bridge technique at the Affiliated Xuzhou Municipal Hospital of Xuzhou Medical University from June 2021 to December 2023.The cohort comprised 49 males and 83 females,aged 18-35(27.4±7.5)years,including 28 cases of mild ptosis and 104 cases of moderate ptosis.During surgery,the levator aponeurosis-Müller′s muscle composite flap was anchored to the tarsal plate using mattress sutures and reinforced with three-layer fixation to optimize force transmission for eyelid elevation.Postoperative follow-up was conducted for 10(6,12)months.Marginal reflex distance-1(MRD1)was measured preoperatively and postoperatively to evaluate aesthetic outcomes,complication rates,and recurrence rates.ResultsThe preoperative and postoperative mean MRD1 values were(1.3±0.3)and(3.6±0.4)mm,respectively,showing a statistically significant difference(P<0.001).In aesthetic evaluation,120 cases(90.9%)were rated as good,8 cases(6.1%)as fair,and 4 cases(3.0%)as poor.As for complications,there were 13 cases(7.7%)of undercorrection,8 cases(4.7%)of overcorrection,and 11 cases(6.5%)of recurrence.ConclusionsThe floating bridge technique for the treatment of mild to moderate ptosis of upper eyelid is safe and effective.Postoperatively,it restores good motor function and aesthetic appearance of the upper eyelid.
