摘要
目的对某部舰艇人员的疼痛特征进行调查分析,并探索其相关因素,为制订科学有效的疼痛管理策略提供参考。方法以方便抽样法在某部选取973名舰艇人员,对其疼痛特征、疼痛刻板心理、运动恐惧状况以及失眠情况进行调查。结果被调查舰艇人员中,703名(72.3%)有疼痛症状,疼痛程度以中度疼痛居多(56.0%),轻度、重度疼痛分别占比37.1%和6.9%;疼痛类型多为肌肉骨骼疼痛(83.9%)、头痛与颔面痛(30.4%),疼痛部位主要为膝和小腿部(46.5%)、腰背部(40.5%)和肩部(14.1%)。703名疼痛人员疼痛刻板心理得分为(29.60±14.56)分、失眠量表得分为(8.52±5.88)分、恐动症评估简表得分为(19.11±8.09)分。多因素有序Logistic回归分析提示,影响疼痛程度等级的因素有疼痛持续时间、疼痛刻板心理和失眠程度(均P<0.05)。结论舰艇人员疼痛发生率高,疼痛程度等级主要受疼痛持续时间、心理灵活性和失眠程度的影响;应早期进行疼痛治疗干预,减少疼痛持续时间,及时给予心理干预,改善睡眠质量,提升舰艇人员疼痛管理效果。
Objective:To investigate and analyze the characteristics of pain among ship crew members in a certain unit and explore its related factors,so as to provide a reference for developing scientific and effective pain management strategies.Methods:A total of 973 ship crew members were selected by convenience sampling from a certain unit.Their pain characteristics,pain-related psychological inflexibility,kinesiophobia,and insomnia were surveyed.Results:The majority of crew members experienced moderate pain(56.0%),while mild and severe pain accounted for 37.1%and 6.9%,respectively.The predominant pain types were musculoskeletal pain(83.9%)and headache/facial pain(30.4%).The pain prevalence rate was 72.3%,with the main pain sites being the knee/lower leg(46.5%),lower back(40.5%)and shoulder(14.1%).Among the 703 crew members with pain,the psychological inflexibility score was 29.60±14.56,the insomnia score was 8.52±5.88,and the Tampa Scale for Kinesiophobia-11 score was 19.11±8.09.Multivariate ordinal regression analysis indicated that pain duration,psychological inflexibility,and insomnia severity were significant factors influencing pain intensity level(all P<0.05).Conclusion:The prevalence rate of pain among ship crew members is high,and pain intensity is primarily influenced by pain duration,psychological flexibility,and insomnia severity.Early pain intervention should be implemented to reduce pain duration,provide timely psychological support,improve sleep quality,thus enhancing pain management efficacy in ship crew members.
