摘要
目的:利用CT三维重建和多平面重组技术,探讨脊柱畸形与食管裂孔疝的关系。方法:选取2019年8月—2023年6月于临湘市人民医院行胸腹部CT检查发现食管裂孔疝的患者101例,作为实验组,选取同时期于临湘市人民医院放射科行胸腹部CT检查无食管裂孔疝的患者100名,作为对照组。测量脊柱Cobb角、收集体重指数(BMI)等资料,比较两组脊柱畸形的发生率及BMI差异,分析食管裂孔疝发生的危险因素。结果:实验组的BMI、Cobb角测量脊柱侧弯异常率、矢状位胸椎、胸腰段曲度异常率高于对照组,差异有统计学意义(P<0.05);两组间Cobb角测量矢状位腰椎曲度异常率比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,BMI增加、脊柱侧弯异常、矢状位胸椎、胸腰段曲度异常均为影响食管裂孔疝发生的危险因素(P<0.05)。结论:发生脊柱侧弯、胸椎及胸腰段曲度异常均能够增加食管裂孔疝发生的风险,食管裂孔疝患者的BMI较高。
Objective To explore the relationship between spinal deformities and hiatal hernia based on CT.Methods A total of 101 patients diagnosed with hiatal hernia through chest and abdominal CT examinations at Linxiang People's Hospital from August 2019 to June 2023 were selected as an experimental group.A total of 100 patients who underwent chest and abdominal CT scans without esophageal hiatal hernia at the same hospital during the same period were selected as the control group.Spinal Cobb angles were measured,BMI values were collected.The incidence of spinal deformities and BMI differences were compared between groups,and risk factors for hiatal hernia were analyzed.Results The BMI and Cobb angle of the experimental group had higher rates of abnormal spinal curvature and abnormal sagittal thoracic and thoracolumbar curvature than the control group,and the differences were statistically significant(P<0.05);there was no statistically signifificant difference in the abnormal sagittal lumbar curvature rate measured by Cobb angle between the two groups(P>0.05).The results of multivariate Logistic regression analysis showed that increased BMI,abnormal spinal curvature,abnormal sagittal thoracic and thoracolumbar curvature were all risk factors for the occurrence of hiatal hernia(P<0.05).Conclusions Spinal curvature,abnormal thoracic and thoracolumbar curvature can all increase the risk of hiatal hernia occurrence.The BMI value of patients with hiatal hernia were relatively high.
