摘要
目的探讨自拟胃安汤加减联合针刺治疗肝胃气滞型胃脘痛的临床效果。方法队列研究。抽取2021年2月至2024年2月河南中医药大学第三附属医院收治的肝胃气滞型胃脘痛患者62例,按随机数字表法分为对照组与研究组,每组31例。对照组采用常规西医联合针刺治疗,研究组在对照组基础上联合自拟胃安汤加减治疗。比较两组临床疗效,中医证候积分,血清胃肠激素水平[胃动素(MTL)],血清肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)水平及不良反应发生情况。结果研究组总有效率(93.55%,29/31)高于对照组(74.19%,23/31),P<0.05。治疗后,研究组中医证候积分低于对照组(P<0.05)。治疗后,两组血清MTL水平均高于治疗前,且研究组高于对照组(P<0.05)。治疗后,研究组血清TNF-α、IL-6、hs-CRP水平低于对照组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论自拟胃安汤加减与针刺联合治疗可有效提高肝胃气滞型胃脘痛患者胃肠功能,改善临床症状,降低炎症反应,提高临床疗效。
Objective To investigate the clinical effect of self-made Weian decoction combined with acupuncture in the treatment of epigastralgia of liver stomach Qi stagnation type.Methods A total of 62 patients with epigastralgia of liver and stomach Qi stagnation type admitted to the Third Affiliated Hospital of Henan University of Chinese Medicine from February 2021 to February 2024 were selected for the cohort study,and they were divided into the control group and the study group according to the random number table method,with 31 cases in each group.The control group was treated with conventional western medicine combined with acupuncture,and the study group was treated with self-made Weian decoction based on the treatment of the control group.The clinical efficacy,traditional Chinese medicine syndrome scores,serum levels of gastrointestinal hormone assessed by motilin(MTL),levels of serum tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP)and interleukin-6(IL-6),and adverse reactions were compared between the two groups.Results The total effective rate of the study group(93.55%,29/31)was higher than that of the control group(74.19%,23/31),P<0.05.After treatment,the scores of traditional Chinese medicine syndrome in the study group were lower than those in the control group(P<0.05).After treatment,the levels of serum MTL in the two groups were higher than those before treatment,and the study group had higher MTL level than the control group(P<0.05).After treatment,the levels of serum TNF-α,IL-6 and hs-CRP in the study group were lower than those in the control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions The combined treatment of self-made Weian decoction and acupuncture can effectively enhance the gastrointestinal function of patients with epigastralgia of liver and stomach Qi stagnation type,improve clinical symptoms,reduce inflammatory response,and increase clinical efficacy.
