摘要
目的基于客观足底压力数据研究踝足矫形器适配效果定量评估特征参数和方法。方法设计基于柔性电路板(FPC)技术的压力检测装置,采集12名健康儿童和15名下肢功能障碍患儿(在穿戴矫形器前、后)的静态和连续步行状态下的足底压力数据。在步态相位自动识别模型的基础上,对静态、动态时空特征进行提取,并从中筛选出能够表征踝足矫形器步态改善的特征集合。设计可解释增强机算法评估足底压力特征的贡献率,在此基础上联合特征贡献率和特征值进行矫形器适配功效评分。结果实现了步态时相的自动识别划分(准确率>0.85),筛选出了足内外压力占比之差、足弓内压力峰值、包络面积、单支撑期占比、y轴动摇径等可有效表征踝足矫形步态改善的特征参数。结论为矫形器适配评估提供了客观度量,有望作为临床评估的有效补充,提高矫形器适配评估环节的准确性。
Objective Ankle-foot orthoses play a positive role in maintaining ankle-foot stability,correcting deformities,improving gait,and promoting lower limb function recovery.However,currently the evaluation of ankle-foot orthose fitness mainly depends on orthotists'experience and patients'feedback,lacking objective indicators.This study established a quantitative evaluation method for the fitness of orthoses based on objective plantar pressure data.Methods A pressure detection device based on flexible printed circuit board(FPC)technology was designed to collect the plantar pressure data of healthy people and the those of patients with foot deformity pre-and post-orthosis-wearing in static and walking states.On the basis of a gait phase automatic recognition model,static and dynamic spatiotemporal characteristics were extracted from the data,and a feature set capable of characterizing the gait improvement of ankle-foot orthoses was selected.An interpretable boosting machine algorithm was designed to evaluate the contribution rates of foot pressure characteristics.The orthosis fitness score was calculated in reference to the feature contribution rates and feature values.Results Automatic recognition of gait phases were achieved(accuracy>0.85),and feature parameters that can effectively characterize the improvement of ankle-foot orthosis gait were selected,including the difference in the proportion of internal and external foot pressure,the peak value of internal foot arch pressure,the envelope area,the proportion of single support period,and the Y-axis swing radius.Conclusions This study provides an objective method for orthotic fitness evaluation and is expected to serve as an effective supplement to clinical assessment,thus offering a new approach for orthotists and physicians for more accurate orthotic fitness assessment.
