摘要
以广东省深圳市某三甲综合医院儿童耳鼻咽喉科2022年3—12月期间完成择期手术患儿为研究对象,运用精益管理工具分析患儿需求和科室服务能力,现场调查择期手术患儿床位预约现状,绘制流程图,结合“五个为什么”找出根本原因,以患儿价值为导向,消除流程中的阻碍和浪费,优化择期手术患儿床位预约管理,缩短患儿等待入院时间。在科室医疗资源没有增加的情况下,科室手术量提升了22%,床位使用率从80.95%提升至95.15%,平均住院日从3.48 d降至3.18 d,患儿择期手术等待入院时间明显缩短(P<0.001)。精益管理的应用能够缩短择期手术患儿等待入院时间,提高医疗资源的利用率,提升患儿就医体验。
This study focuses on pediatric patients who underwent elective surgery in the Department of Pediatric Otolaryngology in a 3A comprehensive hospital in Shenzhen,Guangdong Province from March to December,2022.Lean management tools are used to analyze the needs of patients and the service capabilities of the department.An on-site investigation is conducted to determine the current status of bed reservations for pediatric patients undergoing elective surgery,and a flowchart is drawn.The root cause is identified based on the"Five Whys",guided by patient value,to eliminate obstacles and waste in the process,optimize the management of bed reservations for pediatric patients undergoing elective surgery,and shorten their waiting time for admission.In the absence of an increase in medical resources in the department,the surgical volume increased by 22%;the bed utilization rate increased from 80.95%to 95.15%;and the average length of hospital stay decreased from 3.48 days to 3.18 days.The waiting time for pediatric patients undergoing elective surgery was significantly shortened(P<0.001).It is concluded that the application of lean management can shorten the waiting time for pediatric patients undergoing elective surgery,improve the utilization of medical resources,and enhance the patient's medical experience.
