摘要
目的探讨富血小板血浆(PRP)宫腔灌注联合中医药治疗对宫寒型不孕症患者子宫动脉血流参数及妊娠情况的影响。方法前瞻性选取2022年7月至2023年12月湖北省荆门市中医医院收治的70例宫寒型不孕症患者作为研究对象,按照随机数字表法分为对照组和治疗组,每组35例。对照组脱落2例,最终纳入33例;治疗组未遵医治疗2例,脱落1例,最终纳入32例。对照组采用中医药(暖宫汤+姜疗)治疗,治疗组在对照组基础上联合PRP宫腔灌注治疗。比较两组宫寒型证候积分、子宫内膜厚度、子宫动脉血流参数[搏动指数(PI)、阻力指数(RI)]、妊娠情况。结果治疗3个月经周期后,两组宫寒型证候积分均低于治疗前,治疗组月经量少积分低于对照组(P<0.05);两组其他积分比较,差异无统计学意义(P>0.05)。治疗3个月经周期后,两组子宫内膜厚度均高于治疗前,且治疗组高于对照组(P<0.05)。治疗3个月经周期后,两组子宫动脉PI和RI均低于治疗前,且治疗组低于对照组(P<0.05)。治疗组生化妊娠率和临床妊娠率均高于对照组(P<0.05)。结论宫寒型不孕症患者采用PRP宫腔灌注联合中医药治疗可有效调节子宫动脉血流,增加子宫内膜厚度,改善症状及妊娠情况。
Objective To investigate the impacts of platelet rich plasma(PRP)uterine cavity infusion combined with traditional Chinese medicine treatment on uterine artery blood flow and pregnancy status in patients with uterine cold type infertility.Methods A total of 70 patients with uterine cold type infertility admitted to Jingmen Hospital of Traditional Chinese Medicine from July 2022 to December 2023 were selected as the research subjects.They were randomly assigned into a control group and a treatment group by random number table method,with 35 cases in each group.2 cases fell out of the control group and were eventually 33 cases were included,while 2 cases did not follow medical treatment and fell out of 1 case in the treatment group,and 32 cases were finally included.The control group was treated with traditional Chinese medicine(Warm Palace Decoction+ginger therapy),while the treatment group received PRP uterine cavity infusion therapy on the basis of the control group.The scores of uterine cold type syndrome,endometrial thickness,uterine artery blood flow[pulsatility index(PI),resistance index(RI)],and pregnancy status were compared between the two groups.Results After 3 menstrual cycles of treatment,the scores of uterine cold type syndrome in both groups were lower than before treatment,the treatment group had lower scores of menstrual flow than the control group(P<0.05),and there was no statistically significant difference in other scores between two groups(P>0.05).After 3 menstrual cycles of treatment,the thickness of endometrium in both groups was higher than before treatment,and the treatment group was higher than the control group(P<0.05).After 3 menstrual cycles of treatment,the PI and RI of the uterine arteries in both groups were lower than before treatment,and the treatment group was lower than the control group(P<0.05).The biochemical pregnancy rate and clinical pregnancy rate of the treatment group were higher than those in the control group(P<0.05).Conclusions PRP uterine cavity infusion combined with traditional Chinese medicine treatment can effectively regulate uterine artery blood flow,increase the endometrium thickness,and ameliorate symptoms and pregnancy status in patients with uterine cold type infertility.
