摘要
脊髓梗死是一种罕见疾病,相关报道认为,动脉粥样硬化是导致脊髓梗死的主要病因,其发病机制与局部脊髓低灌注引起的缺血缺氧密切相关。该病的典型特征是突发的截瘫、四肢无力,且预后欠佳。目前脊髓梗死还没有统一确定的治疗方案。中医学中虽没有“脊髓梗死”的病名,但根据脊髓梗死后四肢无力这一典型的临床表现,可将其归属于中医学“痿证”的范畴。中医对“痿证”的诊治有相对完善的理论认识和丰富的实践经验,认为该病发病与血瘀及五脏亏损密切相关。针灸为中医治疗该病的重要方法之一,具有调理脏腑气血、调和阴阳、舒筋通络等作用。临床发现中医针灸治疗脊髓梗死后突发的四肢瘫痪疗效明显,现报道1例针灸治疗脊髓梗死后四肢瘫痪患者。本例辨证为脾肺气虚,辨病为痿证,病位在肌肉,针刺选穴以手足阳明经穴位为主穴,手足太阴经穴位、足太阳经穴位、督脉穴位等为辅穴,共奏健脾益气、疏经通络之功,还针对患者基础疾病配合使用相关对症药物,以期为临床治疗脊髓梗死后突发四肢瘫痪拓展思路。
Spinal cord infarction is a rare disease,and relevant reports believe that atherosclerosis is the main cause of spinal cord infarction,and its pathogenesis is closely related to ischemia and hypoxia caused by local spinal cord hypoperfusion.The typical characteristics of this disease are sudden paraplegia,limb weakness,and poor prognosis.Currently,there is no unified treatment plan for spinal cord infarction.Although there is no disease name of“spinal cord infarction”in TCM,it can be classified into the category of“impotence syndrome”according to the typical clinical manifestation of limb weakness after spinal cord infarction.TCM has a relatively perfect theoretical understanding and rich practical experience in the diagnosis and treatment of“impotence syndrome”,and believes that the disease is closely related to blood stasis and five viscera deficiencies.Acupuncture and moxibustion is one of the important methods of TCM to treat the disease,with the function of regulating Qi(气)and blood of the Zang-fu organs,harmonizing Yin(阴)and Yang(阳),and relaxing tendons and clearing collaterals.In the process of clinical practice,it is found that TCM acupuncture and moxibustion has obvious curative effects on quadriplegia after spinal cord infarction.One case of acupuncture and moxibustion treatment of quadriplegia after spinal cord infarction is reported.In this case,the syndrome differentiation is spleen-lung Qi deficiency,the disease differentiation is impotence syndrome,and the disease is located in muscle.The acupuncture acupoints selected in this case are hand-foot Yangming meridian acupoints as the main acupoints,acupoints of hand-foot Taiyin meridian,foot Taiyang meridian,and governor meridian are used as auxiliary points,to achieve the effects of invigorating spleen and strengthening Qi,and dredging channels and collaterals.Related symptomatic drugs are used for patients with basic diseases.This case is given to develop ideas for clinical treatment of quadriplegia after spinal cord infarction.
