摘要
目的:探讨MR T2WI高信号环征(T2-rim sign)对高强度聚焦超声(HIFU)消融子宫肌瘤术后1年疗效的影响。方法:回顾性分析2018年10月—2023年10月凉山彝族自治州第一人民医院和南充市中心医院经HIFU治疗且按要求完成随访的406例患者(406个靶肌瘤)的临床资料,根据患者术前磁共振图像将患者分为T2-rim sign组(94例)和无T2-rim sign组(312例)。比较两组患者手术前后的子宫体积、肌瘤体积、症状严重程度评分(SSS)和子宫肌瘤症状-生活质量问卷(UFS-QOL)评分,同时比较两组患者的不良反应及并发症发生情况。结果:术后3个月随访时,两组患者子宫体积、肌瘤体积、UFS-QOL评分比较,差异均有统计学意义(Z=-4.692、-8.361、-2.254,P<0.05)。两组患者均无严重不良反应及并发症发生。结论:T2-rim sign对HIFU治疗子宫肌瘤的安全性及有效性无明显影响,HIFU术后1年,T2-rim sign组患者临床疗效满意。
Objective:To explore the impact of MR T2WI high signal rim sign(T2-rim sign)on the 1-year therapeutic respons⁃es of high intensity focused ultrasound(HIFU)in the treatment of uterine fibroid.Methods:A retrospective study was conducted in 406 patients treated with HIFU from October 2018 to October 2023 in the First People’s Hospital of Liangshan Yi Autonomous Prefecture and Nanchong Central Hospital.According to the exist images of preoperative MR T2WI T2-rim sign,the patients were divided into T2-rim sign group(94 patients)and no T2-rim sign group(312 patients).Uterine volume,uterine fibroids volume,symptoms severity score(SSS)and uterine fibroid symptoms quality of life questionnaire(UFS-QOL)score of two groups were com⁃pared.And adverse reactions and complications were analyzed.Results:3-month follow-up after the operation,there were statisti⁃cally significant differences in uterine volume,fibroid volume,and UFS-QOL score between the two groups(Z=-4.692,-8.361,-2.254;P<0.05).No serious adverse reactions or complications occurred in either group.Conclusion:T2-rim sign has no significant impact on the safety and efficacy of HIFU treatment on fibroids.1 year after HIFU surgery,T2-rim sign group are satisfied with the clinical efficacy.
