应用基于虚拟现实的康复训练提高失智症患者认知能力的效果研究

摘要 目的:观察基于虚拟现实(VR)技术的多感官刺激训练,对失智症患者认知能力和衰弱状态的影响,研究多感官刺激训练对失智症患者的干预效果。方法:选取我院神经内科140例失智症患者作为研究对象,采用随机数字表法将其分为观察组和对照组各70例,对照组采用常规综合康复训练方法,包括结构化训练和感觉统合训练,观察组交叉进行综合康复训练及基于VR技术的多感官刺激训练。对比2组干预前以及干预3个月后中文版蒙特利尔认知评估量表(MoCA)和老年人衰弱评估量表(TFI)评分变化。结果:干预3个月后,2组患者MoCA量表中的视空间与执行能力、抽象思维、注意力、定向力及总分均高于干预前(P<0.05,0.01),且观察组以上评分均高于对照组(P<0.05,0.01)。干预后,2组TFI量表中心理衰弱、社会衰弱、认知衰弱及总分均低于干预前(P<0.05,0.01),且观察组以上评分均低于对照组(P<0.05,0.01)。结论:VR技术的多感官刺激训练可改善患者认知能力,提升失智症患者生理、心理、社会和认知衰弱状态,提高患者生活质量,临床干预效果良好。 Objective:To explore the efficacy of multi-sensory treatment based on virtual reality technology in patients with dementia.Methods:A total of 140 patients with dementia in the Department of Neurology of our hospital were selected as research objects,and they were divided into observation group(n=70)and control group(n=70)by random number table method.The control group was treated with conventional comprehensive rehabilitation training methods,including structured training and sensory integration training.The observation group adopted multi-sensory intervention based on virtual reality technology.The score changes of the Chinese version of the Montreal Cognitive Assessment Scale(MoCA)and the Frailty Assessment Scale(TFI)before and 3 months after intervention were compared between the two groups.Results:After 3 months of intervention,the visual space and executive ability,abstract thinking,attention,orientation and total score of MoCA in 2 groups were higher than before intervention,with statistical significance(P<0.05,0.01),and the above scores in the observation group were higher than those in the control group,with statistical significance(P<0.05,0.01).After 3 months of intervention,the psychological fthenia,social fthenia,cognitive fthenia and total scores of TFI in 2 groups were lower than before intervention,and the above scores in the observation group were lower than those in the control group,with statistical significance(P<0.05,0.01).Conclusion:Multi-sensory treatment based on virtual reality technology can improve the cognitive ability of patients with dementia,reduce the frailty status of patients,have positive significance for patients,and can be used in clinical promotion.
