摘要
目的回顾性点评医院门诊利伐沙班处方,分析不合理处方,保证患者合理用药。方法回顾性分析2023年1月至12月钦州市第二人民医院合理用药监测系统中选取4096张门诊使用利伐沙班治疗的处方,评价其合理性。采用帕累托图法分析造成不合理用药的主要、次要及一般因素。结果4096张门诊使用利伐沙班治疗的处方中,合理处方3592张,不合理处方504张,处方合理率87.70%,不合理率12.30%。神经内科最多(187张,37.10%),其次为心血管内科(155张,30.75%)和脑血管病科(54张,10.71%)。帕累托法分析显示主要不合理类型是临床诊断书写不全(235张,46.63%)、用法用量不适宜(207张,41.07%),一般不合理类型是无适应证用药(23张,4.56%)、遴选药品不适宜(23张,4.56%)和有配伍禁忌或不良相互作用(16张,3.18%)。结论结合帕累托图分析发现医院门诊利伐沙班不合理处方主要体现在临床诊断书写不全、用法用量不适宜两方面,通过专项处方点评可以促进合理用药,提高临床医疗质量,保障患者用药安全。
Objective To retrospectively evaluate prescriptions of Rivaroxaban in outpatient departments,analyze irrational prescriptions,and ensure rational drug use for patients.Methods A retrospective analysis was conducted on 4096 prescriptions for outpatient Rivaroxaban treatment selected from the rational drug use monitoring system of the Second People's Hospital of Qinzhou from January to December 2023 to evaluate their rationality.The Pareto chart method was used to analyze the main,secondary and general factors causing irrational drug use.Results Among 4096 outpatient prescriptions for Rivaroxaban treatment,3592 pages were reasonable prescriptions and 504 pages were unreasonable prescriptions.The rationality rate of prescriptions was 87.70%and the irrationalization rate was 12.30%.Neurology department had the largest number(187 pages,37.10%),followed by cardiology department(155 pages,30.75%)and cerebrovascular disease department(54 pages,10.71%).Pareto method analysis showed that the main types of irrationals were incomplete clinical diagnosis writing(235 pages,46.63%),in appropriate dosage and administration(207 pages,41.07%),and the general types of irrationals were unindicated medication(23 pages,4.56%),inappropriate drug selection(23 pages,4.56%),and incompatibility or adverse interactions(16 pages,3.18%).Conclusion Combined with the Pareto chart analysis,it was found that the unreasonable prescriptions of Rivaroxaban in the hospital outpatient department mainly manifested in two aspects:incomplete clinical diagnosis writing and inappropriate dosage and administration.Through special prescription review,it can promote rational drug use,improve the quality of clinical medical care,and ensure the safety of patients'medication.
