摘要
目的:探讨血尿酸(Uric Acid,UA)、血清胆红素(Total Bilirubin,TBIL)与慢性阻塞性肺病急性加重(Acute Exacerbation of Chronic Obstructive Pulmonary Disease,AECOPD)的关系,判断2者对AECOPD有无预测价值。方法:收集2021年10月—2022年10月在石河子大学第一附属医院呼吸内科住院的158例诊断为AECOPD患者的一般资料、入院第1天及经治疗进入COPD稳定期UA、TBIL数值。采用配对样本t检验对2组UA、TBIL值进行统计分析,以准确描述其水平波动情况。采用二元Logistic回归分析UA和TBIL与AECOPD发病的关系,同时借助ROC曲线判断UA和TBIL联合预测AECOPD的效果,并计算UA和TBIL的最佳截点。结果:治疗前AECOPD组的UA和TBIL水平显著高于治疗后的COPD稳定期组,差异具有统计学意义(P<0.05)。二元Logistic回归分析显示UA和TBIL均与AECOPD的发生有关(P<0.05),提示2者均为影响AECOPD发生的独立因素。ROC曲线分析发现UA和TBIL联合预测AECOPD的效能优于单独使用任一指标,其联合预测价值较高。结论:UA、TBIL可以预测AECOPD,且2者联合预测AECOPD的价值高于单个指标。
Objective:To investigate the relationship between blood uric acid(UA),total bilirubin(TBIL)and acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to determine whether they have any predictive value for the development of AECOPD,and to determine whether they have any predictive value for AECOPD.Methods:General data,values of UA and TBIL on the first day of admission and after treatment into the stable stage of COPD were collected from 158 patients diagnosed with AECOPD who were hospitalized in the Department of Respiratory Medicine of the First Affiliated Hospital of Shihezi University from October 2021 to October 2022,and the values of UA and TBIL on the first day of admission and after treatment into the stable stage of COPD were collected.The UA and TBIL values of the two groups were statistically analyzed using paired samples t-test to accurately describe the fluctuations in their levels.Binary logistic regression was used to analyze the relationship between UA and TBIL and the onset of AECOPD,and also to determine the effect of the combination of UA and TBIL in predicting AECOPD with the help of ROC curves,and to calculate the optimal cut-off point for UA and TBIL.Results:The UA and TBIL levels in the pre-treatment AECOPD group were significantly higher than those in the post-treatment COPD group,and the difference was statistically significant(P<0.05).Binary logistic regression analysis showed that both UA and TBIL were associated with the occurrence of AECOPD(P<0.05),suggesting that both were independent factors influencing the occurrence of AECOPD.ROC curve analysis found that the efficacy of UA and TBIL in combining to predict AECOPD was better than that of using either index alone,and their combined predictive value was higher.Conclusion:UA and TBIL can predict AECOPD,and the combined value of the two in predicting AECOPD is higher than that of individual indicators.
