深圳市某中医医院甲状腺疾病的中医诊疗信息分布情况研究

摘要 目的:分析甲状腺疾病中医诊疗信息分布情况,探究甲状腺疾病发生、发展、转归过程中的影响因素,为临床提供疗效证据。方法:选取2021-2022年在深圳市某中医医院收治的1000名甲状腺疾病患者作为研究对象,使用前期构建的诊疗信息采集平台收集与甲状腺疾病相关的诊疗信息,利用贝叶斯网络、知识图谱等方法分析中医诊疗信息分布情况。结果:研究对象中患甲状腺疾病最常见的中医证型为痰瘀内阻,占19.65%;典型症状如怕热、多汗、手抖、心慌、消瘦、胸闷、气促、突眼主要体现在颈部和甲状腺;夏枯草、桔梗、皂角刺、生黄芩、白芍等药物以清热解毒、疏肝理气、软坚散结为主;肝郁气滞证的决策树AUC值最高,精确度达1.00,显示模型稳定;肝郁脾虚证K近邻的AUC值为0.62,精确度1.00,说明决策树和K近邻模型适用于证候建立,但前者更可靠。结论:深圳市某中医医院甲状腺疾病的证候分布以实证为多,症状分布以虚热症状为主,甲状腺疾病的中药治疗以从肝论治为主,肝郁气滞证的最优决策模型是K近邻法,效果明显较高,且运算效率提升明显。 Objective:To analyze the distribution of traditional Chinese medicine(TCM)diagnosis and treatment information for thyroid diseases,to explore the influencing factors in the process of occurrence,development,and regression of thyroid diseases,so as to provide evidence of therapeutic efficacy for the clinic.Methods:1000 patients with thyroid diseases admitted to a TCM hospital in Shenzhen in 2021-2022 were selected as study subjects.The diagnosis and treatment information related to thyroid diseases was collected using the diagnosis and treatment information collection platform constructed in the preliminary stage,and the intelligent diagnosis model was established using Bayesian network and knowledge graph.Results:The most common TCM evidence type was phlegm and stasis internal obstruction,accounting for 19.65%,typical symptoms such as fear of heat,excessive sweating,shaking hands,panic,emaciation,chest tightness,shortness of breath,and protruding eyes were mainly manifested in the neck and the thyroid gland;the drugs such as Xia Gu Cao,Platycodon grandiflorum,Soapberry,Sheng Huangqin,and Bai Shao were mainly used for clearing heat and removing toxins,dispersing the liver,softening and dispersing hard lumps and knots;the decision tree of liver-depression and qi stagnation evidence had the highest AUC value,with the precision of 1.00,which showed that the model was stable The AUC value of K-nearest neighbor for the liver-depression and spleen-deficiency evidence was 0.62,with an accuracy of 1.00,indicating that the decision tree and K-nearest neighbor models are suitable for evidence establishment,but the former is more reliable.Conclusion:The distribution of symptoms of thyroid diseases in a TCM hospital in Shenzhen City is based on solid evidence,and the distribution of symptoms is based on deficiency-heat symptoms,and the TCM treatment of thyroid diseases is based on the treatment from the liver theory.The optimal decision model for the syndrome of liver-qi stagnation is K-nearest neighbor method,which has higher effect and higher efficiency.
