摘要
目的探讨双胎妊娠期糖尿病(GDM)孕妇不良妊娠结局与发病影响因素,并构建预测模型,探索其预测价值。方法选取石家庄市妇幼保健院2020年1月-2023年12月收治的双胎妊娠期糖尿病孕妇165例为研究组,另选择正常双胎妊娠孕妇165例为对照组。比较2组临床资料、实验室指标、孕妇及新生儿不良结局,采用Logistic回归分析双胎孕妇妊娠期糖尿病发病的危险因素,并构建预测模型分析预测价值。结果孕前体质量指数(BMI)≥28 kg·m^(-2)、年龄≥35岁、糖尿病家族史、多囊卵巢综合征、不良妊娠史及全血三酰甘油(TG)、总胆固醇(TC)、空腹血糖(FPG)水平偏高是双胎孕妇妊娠期糖尿病发病的独立危险因素(P<0.05)。受试者工作特征曲线(ROC)分析显示,曲线下面积(AUC)值为0.893,敏感度为84.85%,特异度为83.03%。结论双胎孕妇发生妊娠期糖尿病易增加不良妊娠结局发生风险,且发病与孕前BMI、年龄、糖尿病家族史、多囊卵巢综合征、不良妊娠史及全血TG、TC、FPG水平有关,据此构建预测模型具有较好的预测价值。
Objective To explore the adverse pregnancy outcomes and the influencing factors of its incidence in twin pregnant women with gestational diabetes mellitus(GDM),and to construct a prediction model to explore its predictive value.Methods 165 twin pregnant women with GDM admitted to Shijiazhuang Maternal and Child Health Hospital from January 2020 to December 2023 were selected as the study group,another 165 pregnant women with normal twin pregnancies were selected as the control group.Clinical data,laboratory indicators,adverse outcomes of pregnant women and newborns were compared between two groups.The risk factors for the incidence of GDM in twin pregnant women were analyzed by Logistic regression,and a prediction model was constructed to analyze its predictive value.Results Pre-pregnancy body mass index(BMI)≥28 kg·m^(-2),age≥35 years,family history of diabetes mellitus,polycystic ovary syndrome,history of adverse pregnancies,and high levels of whole blood triglyceride(TG),total cholesterol(TC)and fasting plasma glucose(FPG)were the independent risk factors for the incidence of GDM in twins pregnancies(P<0.05).Receiver operating characteristic curve(ROC curve)analysis showed that area under the curve(AUC)value was 0.893,the sensitivity was 84.85%,and the specificity was 83.03%.Conclusion The occurrence of GDM in twin pregnancies is likely to increase the risk of adverse pregnancy outcomes,and the incidence is related to BMI before pregnancy,age,family history of diabetes mellitus,polycystic ovary syndrome,history of adverse pregnancies,and the levels of whole blood TG,TC and FPG.The prediction model constructed based on this has a better predictive value.
