摘要
目的:探讨糖尿病性白内障(DC)患者房水中血管内皮生长因子(VEGF)、白细胞介素-6(IL-6)、转化生长因子-β1(TGF-β1)、基质金属蛋白酶-9(MMP-9)水平变化及临床意义。方法:选取2022年2月—2024年2月某院收治的DC患者118例为观察组,另选取同期就诊的单纯白内障患者30例为对照组,依据DC分期将观察组分为初始期(n=34)、肿胀期(n=30)、成熟期(n=42)及过熟期(n=12),比较观察组和对照组、不同分期DC患者房水中VEGF、IL-6、TGF-β1、MMP-9水平,采用Spearman系数分析各指标与DC分期的相关性,绘制ROC曲线分析房水中VEGF、IL-6、TGF-β1、MMP-9水平对DC的诊断效能。结果:观察组房水中VEGF、IL-6、TGF-β1、MMP-9水平均高于对照组,差异有统计学意义(P<0.05)。不同DC分期患者房水中VEGF、IL-6、TGF-β1、MMP-9水平比较:初始期<肿胀期<成熟期<过熟期,差异有统计学意义(P<0.05)。相关性分析显示,房水中VEGF、IL-6、TGF-β1、MMP-9水平与DC分期呈正相关(r=0.466、0.437、0.421、0.433,P均<0.05)。房水中VEGF、IL-6、TGF-β1、MMP-9诊断DC的AUC分别为0.820、0.881、0.791、0.888;四者联合诊断的AUC为0.983,灵敏度为95.33%,特异度为96.67%。结论:DC患者房水中VEGF、IL-6、TGF-β1、MMP-9水平异常升高,且与DC分期呈正相关,联合检测其水平具有较高的诊断效能。
Objective:To investigate the expression and significance of vascular endothelial growth factor(VEGF),interleukin-6(IL-6),transforming growth factorβ1(TGF-β1),and matrix metalloproteinase-9(MMP-9)in atrial fluid in patients with diabetic cataracts(DC).Methods:118 patients with DC who were treated in a hospital from February 2022 to February 2024 were selected as the observation group,and 30 patients with simple cataracts during the same period were chosen as the control group.The differences in the levels of VEGF,IL-6,TGF-β1,and MMP-9 in the atrial fluid of the patients in the two groups were detected and compared.The patients of the observation group were classified into the initial stage(n=34),the swelling stage(n=30),the maturity stage(n=42),and the over-maturity stage(n=12).The differences in the levels of VEGF,IL-6,TGF-β1,and MMP-9 in the patients of the two groups with different DC stages were compared.Spearman was used to analyze the relationship between the indicators and DC staging,and ROC curve was plotted to analyse the value of VEGF,IL-6,TGF-β1,and MMP-9 in the diagnosis of DC.Results:The levels of VEGF,IL-6,TGF-β1,and MMP-9 in patients in the observation group were higher than those in the control group(P<0.05).In comparison of the levels of VEGF,IL-6,TGF-β1,and MMP-9 in the atrial fluid of DC patients,the initial stage<the swelling stage<the mature stage<the over mature stage(P<0.05).Pearson analysis showed that VEGF,IL-6,TGF-β1,and MMP-9 were positively correlated with DC staging(r=0.466,0.437,0.421,0.433,P<0.05).The AUC of VEGF,IL-6,TGF-β1,and MMP-9 for diagnosing DC were 0.820(95%CI 0.749~0.879),0.881(95%CI 0.817-0.928),0.791(95%CI 0.717~0.853),and 0.888(95%CI 0.826~0.934),respectively.The AUC of the combined diagnosis of DC by the four was 0.983(95%CI 0.947~0.997),with a sensitivity of 95.33%and specificity of 96.67%.Conclusion:The levels of VEGF,IL-6,TGF-β1,and MMP-9 were abnormally elevated in the atrial fluid of DC patients,and the levels of the above indexes were positively related to the DC disease stage,which is of great value in the diagnosis of the disease.
