摘要
目的观察肾衰宁颗粒联合复方α-酮酸片治疗慢性肾衰竭的疗效,并探讨其对患者微炎症状态及营养状况的影响。方法选择2022年1月至2024年5月普宁市人民医院收治的60例慢性肾衰竭患者作为研究对象,按随机数表法分为观察组和对照组,每组各30例。两组患者均接受常规治疗,在常规治疗基础上对照组患者予以复方α-酮酸片治疗,观察组患者在对照组治疗的基础上联合肾衰宁颗粒治疗,均持续治疗4周。比较两组患者治疗4周后的临床疗效,以及治疗前后的尿素氮(BUN)、血肌酐(Scr)、估算的肾小球滤过率(eGFR)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、血红蛋白(Hb)、白蛋白(ALB)、前白蛋白(PA)水平,并比较两组患者治疗期间的不良反应发生率。结果治疗后,观察组患者的总有效率为90.00%,明显高于对照组的66.67%,差异有统计学意义(P<0.05);治疗后,观察组患者的BUN、Scr分别为(14.23±1.85)mmol/L、(405.28±37.36)μmol/L,明显低于对照组的(16.52±1.49)mmol/L、(430.51±34.09)μmol/L,eGFR为(41.66±3.28)mL/(min·1.73 m2),明显高于对照组的(38.97±3.45)mL/(min·1.73 m2),差异均有统计学意义(P<0.05);治疗后,观察组患者的CRP、TNF-α、IL-6水平分别为(12.47±2.50)mg/L、(52.41±6.38)ng/L、(33.21±3.17)ng/L,明显低于对照组的(16.59±2.74)mg/L、(61.25±5.40)ng/L、(39.47±3.60)ng/L,差异均有统计学意义(P<0.05);治疗后,观察组患者的Hb、ALB、PA水平分别为(107.23±7.49)g/L、(40.84±3.72)g/L、(335.27±25.86)mg/L,明显高于对照组的(101.56±8.47)g/L、(37.26±2.94)g/L、(312.20±21.63)mg/L,差异均有统计学意义(P<0.05);治疗期间,观察组和对照组患者的不良反应总发生率分别为13.33%、6.67%,差异无统计学意义(P>0.05)。结论肾衰宁颗粒联合复方α-酮酸片治疗慢性肾衰竭的疗效明显,并有助于改善患者微炎症状态、营养状况及肾功能,具有临床应用价值。
Objective To observe the clinical efficacy of Shenshuaining Granules combined with Compoundα-Ketoacid Tablets in treating chronic renal failure(CRF),and to explore their impact on the micro-inflammatory state and nutritional status of patients.Methods A total of 60 patients with chronic renal failure admitted to Puning People's Hospital from January 2022 to May 2024 were selected and divided into an observation group and a control group ac-cording to the random number table method,with 30 cases in each group.All patients in both groups received conven-tional treatment.On this basis,patients in the control group were treated with Compoundα-Ketoacid Tablets,whereas those in the observation group received Shenshuaining Granules in addition to the treatment given to the control group.Treatment lasted 4 weeks for both groups.After 4 weeks,clinical efficacy was assessed and the following parameters were compared before and after treatment:blood urea nitrogen(BUN),serum creatinine(Scr),estimated glomerular fil-tration rate(eGFR),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),hemoglobin(Hb),albumin(ALB),and prealbumin(PA).Additionally,the incidence of adverse reactions during the treatment period was compared between the two groups.Results After treatment,the total effective rate in the observation group was 90.00%,significantly higher than 66.67%in the control group(P<0.05).After treatment,the levels of BUN and Scr in the observation group were(14.23±1.85)mmol/L and(405.28±37.36)μmol/L,respectively,both significantly lower than corresponding(16.52±1.49)mmol/L and(430.51±34.09)μmol/L in the control group(P<0.05);the estimated eGFR in the observation group was(41.66±3.28)mL/(min·1.73 m2),significantly higher than(38.97±3.45)mL/(min·1.73 m2)in the control group(P<0.05);the levels of CRP,TNF-α,and IL-6 in the observation group were(12.47±2.50)mg/L,(52.41±6.38)ng/L,and(33.21±3.17)ng/L,respectively,all significantly lower than corresponding(16.59±2.74)mg/L,(61.25±5.40)ng/L,and(39.47±3.60)ng/L in the control group(P<0.05);the levels of Hb,ALB,and PA in the observa-tion group were(107.23±7.49)g/L,(40.84±3.72)g/L,and(335.27±25.86)mg/L,respectively,all significantly higher than corresponding(101.56±8.47)g/L,(37.26±2.94)g/L,and(312.20±21.63)mg/L in the control group(P<0.05).Dur-ing the treatment,the total incidence of adverse reactions was 13.33%in the observation group and 6.67%in the control group,with no statistically significant difference(P>0.05).Conclusion Shenshuaining Granules combined with Com-poundα-Ketoacid Tablets produce a pronounced therapeutic effect in chronic renal failure,improving the patients'mi-cro-inflammatory status,nutritional condition,and renal function without increasing adverse reactions.The regimen has promising clinical application value.
