羟苯磺酸钙联合骨化三醇对糖尿病肾病患者氧化应激、微炎症状态及冠状动脉钙化的影响

Effects of Calcium Dobesilate Combined with Calcitriol on Oxidative Stress,Microinflammatory Status and Coronary Artery Calcification in Diabetic Nephropathy Patients

摘要 目的观察羟苯磺酸钙联合骨化三醇对糖尿病肾病(DN)患者氧化应激、微炎症状态及冠状动脉钙化的影响。方法选取2023年5月至2024年3月新疆医科大学第一附属医院收治的DN患者50例,按照信封随机法分为单一用药组(仅采用骨化三醇)与联合用药组(采用羟苯磺酸钙联合骨化三醇),每组25例。比较两组肾功能[血尿素氮(BUN)、血肌酐(Scr)]、氧化应激[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]、微炎症状态[超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)]、冠状动脉钙化积分(CACS)等指标水平及不良反应发生情况。结果治疗后,联合用药组BUN[(4.13±0.87)mmol/L]、Scr[(71.84±7.51)μmol/L]、hs-CRP[(5.92±1.01)mg/L]、IL-6[(0.42±0.05)ng/L]、TNF-α[(6.32±0.89)μg/L]、CACS[(64.28±4.97)分]低于单一用药组[(4.81±1.01)mmol/L,(79.26±8.25)μmol/L、(6.81±1.09)mg/L、(0.51±0.09)ng/L、(6.98±1.04)μg/L,(72.31±5.23)分],SOD[(126.21±11.81)U/mL]、GSH-Px[(121.43±12.06)U/L]水平高于单一用药组[(107.65±10.72)U/m L,(112.59±11.48)U/L],差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论羟苯磺酸钙与骨化三醇联合应用对DN患者肾功能、氧化应激、微炎症状态均有显著改善作用,还可延缓冠状动脉钙化,安全性良好。 Objective To observe the effect of calcium dobesilate combined with calcitriol on oxidative stress,microinflammatory status,and coronary artery calcification in diabetic nephropathy(DN)patients.Methods Fifty patients with DN admitted to the First Affiliated Hospital of Xinjiang Medical University from May 2023 to March 2024 were selected.Using the sealed envelope randomization method,the subjects were randomly assigned to the monotherapy group(calcitriol alone,25 cases)and the combined therapy group(calcium dobesilate combined with calcitriol,25 cases).Comparison between the two groups was made on renal function[blood urea nitrogen(BUN)and serum creatinine(Scr)],oxidative stress[superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)],microinflammatory status[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)],coronary artery calcium score(CACS),and adverse reactions.Results After treatment,the levels of BUN[(4.13±0.87)mmol/L],Scr[(71.84±7.51)μmol/L],hs-CRP[(5.92±1.01)mg/L],IL-6[(0.42±0.05)ng/L],TNF-α[(6.32±0.89)μg/L],and CACS[(64.28±4.97)points]in the combination therapy group were significantly lower than those in the monotherapy group[(4.81±1.01)mmol/L,(79.26±8.25)μmol/L,(6.81±1.09)mg/L,(0.51±0.09)ng/L,(6.98±1.04)μg/L,and(72.31±5.23)points,respectively].The levels of SOD[(126.21±11.81)U/mL]and GSH-Px[(121.43±12.06)U/L]in the combination therapy group were higher than those in the monotherapy group[(107.65±10.72)U/mL and(112.59±11.48)U/L,respectively],and the differences were statistically significant(P<0.05).There was no statistically significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion The combined use of calcium dobesilate and calcitriol can significantly improve renal function,oxidative stress,and microinflammatory status in DN patients,while also delaying coronary artery calcification,with good safety.
