摘要
卒中后癫痫是最常见的继发性癫痫之一,脑卒中与癫痫往往相互影响,若脑卒中反复发生癫痫,可影响神经元的修复,显著加重病情,影响预后。现代医学将其发病归咎于卒中后脑组织缺血缺氧以及遗传易感等因素,治疗上则是应用抗癫痫药物以及疾病修饰等疗法,中医学则围绕“风、火、痰、瘀、虚”五大病理因素处方用药。风引汤出自《金匮要略》,“除热瘫痫”高度概括了本方所治之症,方中所用介石类药占四分之三,然此方不仅重镇熄风,更有燮理阴阳、平调寒热、斡旋中气、填塞空窍之功,深入研究其方义对于治疗卒中后癫痫有着重要的指导意义。
Post-stroke epilepsy is one of the most common secondary epilepsy,and stroke and epilepsy often interact with each other.If epilepsy occurs repeatedly after stroke,it can affect the repair of neurons,significantly aggravating the illness conditions and affecting its prognosis.Modern medicine attributes the disease to cerebral tissue ischemia and hypoxia after stroke and genetic susceptibility and other factors,and treats it with anti-epileptic drugs and the disease modification therapy,while traditional Chinese medicine decides on appropriate prescriptions and medication based on the five pathological factors of"wind,fire,phlegm,blood stasis,and deficiency".Fengyin Decoction is derived from the Synopsis of the Golden Chamber,whose activity is summarized as"eliminating heat,paralysis,and epilepsy",precisely denoting the symptoms treated by the prescription.With three quarters of the constituents being mineral drugs in it,the prescription is primarily used to suppress hyperactive liver for calming endogenous wind,and it also has the functions of regulating yin and yang,mildly regulating cold and heat,mediating middle qi,and filling up sweat pores and striae.This study explores the connotations of Fengyin Decoction in a bid to give full play to its great guiding significance for the treatment of post-stroke epilepsy.
