颈动脉斑块内新生血管分级对缺血性脑卒中风险的评估价值分析

Value of carotid plaque neovascularization grading in predicting the risk of ischemic stroke

摘要 目的分析超声检测颈动脉斑块内新生血管分级对缺血性脑卒中风险的评估价值。方法回顾性选取2022年6月至2024年6月宁波市第四医院收治的缺血性脑卒中合并颈动脉硬化斑块患者35例为研究组,另选取同期收治的仅有颈动脉硬化斑块患者25例作为对照组。采用超声检测入选者颈动脉硬化斑块中的新生血管分级,比较两组临床资料,采用Pearson相关性分析D-二聚体水平、斑块内新生血管分级与缺血性脑卒中患者发病的相关性。应用logistic回归剖析缺血性脑卒中发病的多重风险因素。结果研究组的D-二聚体水平、斑块内新生血管分级Ⅲ级、Ⅳ级占比分别为(845.68±101.02)μg/L、51.4%(18/35)、31.4%(11/35),均高于对照组的(446.89±103.25)μg/L、24.0%(6/25)、8.0%(2/25),研究组斑块内新生血管分级Ⅱ级占比为5.7%(2/35),低于对照组的36.0%(9/25),两组差异均有统计学意义(t=7.82,χ^(2)=8.93、4.57、4.72,P<0.001、P=0.003、0.033、0.030);Pearson相关性检验显示,D-二聚体水平、颈动脉斑块新生血管分级与缺血性脑卒中患者发病均呈正相关(r=0.825、0.826,均P<0.05)。多因素logistic回归分析显示,D-二聚体水平、颈动脉斑块新生血管分级为影响缺血性脑卒中患者发病的危险因素(OR=3.414、3.449,均P<0.05)。结论D-二聚体水平、颈动脉斑块新生血管分级在预测缺血性脑卒中风险中均有较高的价值。 ObjectiveTo analyze the value of carotid plaque neovascularization grading in predicting the risk of ischemic stroke.MethodsThirty-five patients with ischemic stroke and carotid atherosclerotic plaques,admitted to Ningbo No.4 Hospital from June 2022 to June 2024,were retrospectively included in the study group.Additionally,25 patients with only carotid atherosclerotic plaques admitted during the same period were included in the control group.Ultrasound examination was performed to assess the neovascularization grading in the carotid atherosclerotic plaques of the enrolled patients.The clinical data of both groups were compared.Pearson's correlation analysis was performed to analyze the relationship between D-dimer levels,neovascular grading in the plaques,and the onset of ischemic stroke.Logistic regression was applied to identify multiple risk factors for the onset of ischemic stroke.ResultsD-dimer levels and the percentages of gradesⅢandⅣneovascularization in the study group were(845.68±101.02)μg/L,51.4%(18/35),and 31.4%(11/35),respectively.These values were significantly higher than those in the control group[(446.89±103.25)μg/L,24.0%(6/25),8.0%(2/25)].The percentage of gradeⅡneovascularization in the study group was significantly lower than that in the control group[5.7%(2/35)vs.36.0%(9/25)].All differences between the two groups were statistically significant(t=7.82,χ^(2)=8.93,4.57,4.72,P<0.001,P=0.003,P=0.033,P=0.030).Pearson's correlation test indicated a positive correlation between D-dimer level,carotid plaque neovascularization grade,and the onset of ischemic stroke(r=0.825,0.826,both P<0.05).Multivariate logistic regression analysis showed that D-dimer level and carotid plaque neovascularization grade were significant risk factors associated with the incidence of ischemic stroke(OR=3.414,3.449,both P<0.05).ConclusionsD-dimer level and carotid plaque neovascularization grade have high value in predicting the risk of ischemic stroke.
