MIIP表达与肝细胞癌血管生成、侵袭转移及预后的关系

Relationship between MIIP expression and angiogenesis,invasive metastasis and prognosis of hepatocellular carcinoma

摘要 目的检测迁移侵袭抑制蛋白(migration and invasion inhibitory protein,MIIP)在肝细胞癌(hepatocellular carcinoma,HCC)中的表达,分析其与HCC血管生成、侵袭转移及预后的关系。方法应用免疫组化检测89例手术治疗的HCC患者癌组织及癌旁组织中MIIP、CD34及E-cadherin、N-cadherin的表达,计算微血管密度(microvessel density,MVD),ELISA法检测HCC患者血清VEGFA含量,术后随访患者3年。分析MIIP表达与血清VEGFA、MVD、上皮细胞-间充质转化(epithelial-mesenchymal transition,EMT)及临床病理参数、术后预后的关系。结果癌组织MIIP表达量显著低于癌旁组织(P<0.01);MIIP低表达组血清VEGFA含量、MVD均显著高于MIIP高表达组(P<0.01);癌组织MIIP表达量与E-cadherin表达呈正相关(r=0.403,P<0.001),与N-cadherin呈负相关(r=-0.443,P<0.001),MIIP表达量与肿瘤分化程度、肿瘤大小、CNLC分期、淋巴结转移、血管侵犯、术后复发转移显著相关(P<0.01)。Kaplan-Meier分析显示MIIP低表达患者术后2、3年生存率显著低于MIIP高表达患者(P<0.01)。单因素及多因素分析显示MIIP是HCC预后的独立危险因素。结论MIIP表达量降低可促进HCC新生血管生成、EMT和侵袭转移,与不良预后显著相关,可成为HCC临床预后的评价指标。 Objective To detect the expression of migration and invasion inhibitory protein(MIIP)in hepatocellular carcinoma(HCC)and analyze its relationship with angiogenesis,invasive metastasis and prognosis of HCC.Methods The expressions of MIIP,CD34,E-cadherin and N-cadherin in cancer tissues and adjacent tissues of 89 patients with HCC were detected by immunohistochemistry.The microvessel density(MVD)was calculated,and the serum VEGFA content of HCC patients was detected by ELISA method.The patients were followed up for 3 years.The relationship between MIIP expression and serum VEGFA,MVD,epithelial-mesenchymal transition(EMT),clinicopathological parameters and postoperative prognosis were analyzed.Results The expression of MIIP in cancer tissues was significantly lower than that in paracancerous tissues(P<0.01).The serum VEGFA content and MVD in MIIP low expression group were significantly higher than those in MIIP high expression group(P<0.01).The expression of MIIP was positively correlated with the expression of E-cadherin(r=-0.403,P<0.01)and negatively correlated with N-cadherin(r=-0.443,P<0.001).The expression of MIIP was significantly correlated with tumor differentiation,tumor size,CNLC stage,lymph node metastasis,vascular invasion,postoperative recurrence and metastasis(P<0.01).Kaplan-Meier analysis showed that the 2-and 3-year survival rate of patients with low expression of MIIP was significantly lower than that of patients with high expression of MIIP(P<0.01).Univariate and multivariate analysis showed that MIIP was an independent risk factor for the prognosis of HCC.Conclusions Decreased expression of MIIP can promote angiogenesis,EMT,invasion and metastasis of HCC,which is significantly related to adverse prognosis and can be used as an index to evaluate the clinical prognosis of HCC.
