新生儿肺出血临床特征分析及机械通气对策研究

摘要 分析新生儿肺出血临床特征,探究机械通气方式。选择南宁市第一人民医院新生儿科2015年1月—2024年12月收治的肺出血患儿作为研究对象,分析该病的临床特征,根据肺出血后机械通气治疗方式将患儿分为常频通气组(20例)和高频振荡通气组(23例)进行比较,探究影响机械通气方式的因素。结果显示,43例肺出血患儿中早产儿31例(72.09%),起病时间为出生后1 h~23天(中位时间为1.8天),常见的前3位病因或并发症为呼吸窘迫综合征、凝血功能障碍、动脉导管未闭。经治疗,9例患儿死亡(病死率为20.93%),不同胎龄及体重肺出血患儿的病死率比较,无统计学差异(P>0.05)。高频振荡通气组患儿的中位发病时间显著早于常频通气组(P<0.05)。研究发现,新生儿肺出血常见于早产儿,病死率高,发病时间可作为床旁评估患儿病情危重程度的简易指标。对于出生24 h内发病的患儿,需高度警惕常规通气失败的风险,可首选高频通气治疗。 To analyze the clinical characteristics of neonatal pulmonary hemorrhage and explore the countermeasures for mechanical ventilation mode selection.The children with pulmonary hemorrhage admitted to the Department of Neonatology of The First People’s Hospital of Nanning from January 2015 to December 2024 were selected as the research objects.The clinical characteristics of the disease were analyzed,and they were divided into conventional ventilation group(20 cases)and high-frequency oscillatory ventilation group(23 cases)according to the mechanical ventilation treatment mode after pulmonary hemorrhage to explore the factors affecting the mechanical ventilation mode.The results showed that among 43 children with pulmonary hemorrhage,31 were premature infants(72.09%).The onset time was 1 hour to 23 days after birth(median time was 1.8 days).The top three common causes or complications were respiratory distress syndrome,coagulation dysfunction,and patent ductus arteriosus.After treatment,9 children died(mortality rate was 20.93%).There was no statistical difference in the mortality rate of children with pulmonary hemorrhage of different gestational ages and weights(P>0.05).The median onset time of children in the high-frequency oscillatory ventilation group was significantly earlier than that in the conventional ventilation group(P<0.05).The study found that neonatal pulmonary hemorrhage is common in premature infants with a high mortality rate.The onset time can be used as a simple index for bedside assessment of the severity of children’s condition.For children who develop the disease within 24 hours after birth,the risk of conventional ventilation failure needs to be highly alerted,and high-frequency ventilation can be preferred.
