摘要
探讨采用柴胡加龙骨牡蛎汤治疗肝胃郁热型胃食管反流病的临床效果。纳入2022年1月—2024年12月柳州市中医医院收治的81例肝胃郁热型胃食管反流病患者为研究对象,随机将其分为对照组(n=40,实施常规西医方案治疗)和研究组(n=41,在对照组的基础上联合柴胡加龙骨牡蛎汤加减治疗)。治疗前后分别对两组患者的食管黏膜状态、食管动力学参数进行检测,并采用QOL-30量表和PSQI评分评估其生活质量与睡眠质量改善情况。结果显示,治疗后,与对照组相比,研究组在食管功能改善方面呈现显著优势。研究组食管黏膜损伤评分低于对照组,而食管下端括约肌静息压力与远端收缩积分指标则显著高于对照组(P<0.05)。研究组PSQI睡眠质量评分低于对照组,同时其QOL-30生活质量评分高于对照组(P<0.05)。研究发现,对于肝胃郁热型胃食管反流病,采用柴胡加龙骨牡蛎汤加减治疗能显著改善患者症状,调节食管动力功能,提高患者的睡眠质量,具有较高的临床应用价值。
To explore the clinical effect of Chaihu Jia Longgu Muli Decoction in the treatment of gastroesophageal reflux disease(GERD)of liver-stomach stagnant heat type.A total of 81 patients with liver-stomach stagnant heat-type GERD admitted to Liuzhou Traditional Chinese Medicine Hospital from January 2022 to December 2024 were enrolled and randomly divided into a control group(n=40,treated with conventional western medicine)and an research group(n=41,treated with Chaihu Jia Longgu Muli Decoction plus conventional western medicine).Before and after treatment,esophageal mucosal status and esophageal dynamic parameters were detected in both groups.The quality of life(QOL-30 scale)and sleep quality(PSQI score)were evaluated to assess improvement.The results showed that after treatment,the research group demonstrated significant advantages in esophageal function improvement compared with the control group.The esophageal mucosal injury score in the research group was lower,while the lower esophageal sphincter resting pressure and distal contraction integral were significantly higher than those in the control group(P<0.05).The PSQI sleep quality score of the research group was lower,and the QOL-30 quality of life score was higher than those of the control group(P<0.05).It is concluded that Chaihu Jia Longgu Muli Decoction plus conventional treatment can significantly improve symptoms,regulate esophageal motility,and enhance sleep quality in patients with liver-stomach stagnant heat-type GERD,showing high clinical application value.
