摘要
目的:使用数据挖掘及网络药理学研究方法探究何泽云教授治疗肾病综合征的用药规律及核心药对作用机制。方法:收集2018年9月-2024年4月湖南中医药大学第一附属医院何泽云教授门诊收治的肾病综合征患者的病案,利用Excel进行整理,将数据录入古今医案云平台V2.3.7,分析患者的年龄及性别,统计中药使用频次、性味归经,并进行药物关联规则分析、聚类分析、复杂网络分析等,获取核心药对,通过网络药理学分析其作用机制。结果:共获得中药处方680首,涉及单味中药174种,中药总使用频次为28520。核心药物有山药、白茅根、桑叶、黄精、麦冬等。药性以寒性、平性为主,药味以甘为主,归经以肺经、脾经、胃经、肾经为主。通过关联规则分析获得核心药对为山药–白茅根,通过聚类分析得到3个新方组合,通过复杂网络分析获得1个核心处方。以数据挖掘结果得出核心药对(山药–白茅根)为研究对象,网络药理学分析表明,表皮生长因子受体(Epidermal Growth Factor Receptor,EGFR)、Src酪氨酸蛋白激酶(SRC)、肿瘤坏死因子(Tumor Necrosis Factor,TNF)等靶基因度值较高,山药–白茅根通过癌症信号通路、丝裂原活化蛋白激酶(Mitogen–activated Protein Kinase,MAPK)信号通路、磷脂酰肌醇3激酶(Phosphatidylinositol 3–kinase,PI3K)–蛋白激酶B(Akt)信号通路等发挥治疗肾病综合征的作用。结论:何泽云教授治疗肾病综合征的核心药对为山药–白茅根,该药对治疗肾病综合征的机制与调节炎症反应和免疫密切相关。
Objective:Through data mining and network pharmacology,the medication patterns of Professor HE Zeyun in the treatment of nephrotic syndrome and the action mechanism of core medicine pairs were explored in this study.Methods:The medical records of nephrotic syndrome patients admitted to the outpatient department of Professor HE Zeyun in the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from September 2018 to April 2024 were collected.Relevant data were sorted out by Excel and entered into the ancient and modern medical reports cloud platform V2.3.7 to analyze the age and gender of the patients,as well as the usage frequency,medicinal properties and tastes,and tropism meridian of Chinese medicines.The association rule analysis,clustering analysis,and complex network analysis of Chinese medicines included were performed to obtain core medicine pairs,so as to analyse their action mechanisms by network pharmacology.Results:A total of 680 TCM prescriptions were obtained,involving 174 Chinese medicines,and the total frequency was 28520.Core medicines were Shanyao(Rhizoma Dioscoreae),Baimaogen(Rhizoma Imperatae),Sangye(Mulberry Leaf),Huangjing(Rhizoma Polygonati),and Maidong(Radix Ophiopogon).The medicinal properties were mainly cold and moderate properties,the medicinal taste was mainly sweet,and the tropism meridians were mainly lung meridian,spleen meridian,stomach meridian,and kidney meridian.The core medicine pair of Shanyao-Baimaogen was yielded by medicine association rule analysis,3 new prescription combinations were obtained by cluster analysis,and 1 core prescription was obtained by complex network analysis.The network pharmacological analysis showed that target genes such as EGFR,SRC,and TNF had high degree values,and Shanyao-Baimaogen could play a role in the treatment of nephrotic syndrome through signaling pathway in cancer,MAPK signaling pathway,and PI3K-Akt signaling pathway mainly.Conclusion:The core medicine pair used by Professor HE Zeyun in the treatment of nephrotic syndrome is Shanyao-Baimaogen.The action mechanism of the Shanyao-Baimaogen medicine pair in the treatment of nephrotic syndrome was closely related to the regulation of inflammatory response and immunity.
