摘要
心房颤动是临床最常见的心律失常之一,致病原因复杂多样,年龄增长、自身基础疾病、不良的生活方式、家族遗传、情绪、心理状态等因素均会增加心房颤动的易感性,使心房颤动的发作更加频繁、发作时间延长。心房颤动的病程较长,且有反复发作的趋势,容易诱发其他并发症,显著增加病死、脑卒中、心力衰竭、认知功能障碍、痴呆等的风险,临床治疗难度较大,患者的生活质量较差、生活负担较重。随着中医体系日益完善,其治疗心房颤动具有独特的优势,分为中医内治法和外治疗法。传统内治疗法主要通过辨证论治对中药进行加减化裁,遵循一人一方原则,定期随诊。中医外治法包括针刺、穴位贴敷、耳穴压豆等,患者接受度较高且依从性良好,生活质量得到改善。本研究在传统中医辨证论治的前提下,从风、火、痰、瘀、虚等角度切入,以“息风镇静,通络定悸”“清热化火,复脉定悸”“活血化瘀,祛痰定悸”“益气补虚,稳心定悸”等为原则对近几年心房颤动的病因病机、辨证分型、中医辨证治疗进展等进行综述,为中医治疗心房颤动提供思路。
Atrial fibrillation is one of the most common arrhythmias in clinical practice.The pathogenic causes of atrial fibrillation are complex and diverse.Age,underlying diseases,poor lifestyle,family heredity,emotion,and psychological state will increase the susceptibility to atrial fibrillation,making the attack of atrial fibrillation more frequent and prolonged.Atrial fibrillation has a long course of disease and a tendency of repeated attacks,which is easy to induce other complications and significantly increase the risk of death,stroke,heart failure,cognitive dysfunction,dementia,etc.,is more difficult to treat clinically,and patients have poor quality of life and heavy burden of life.The traditional Chinese medicine system is increasingly improved,and shows unique advantages in the treatment of atrial fibrillation,mainly including internal therapy and external therapy of TCM.Traditional internal therapy mainly adds or subtracts Chinese medicines according to syndrome differentiation,follows the principle of one prescription for one patient,and regularly follows up.The traditional external therapies in TCM include acupuncture,acupoint application,auricular acupoint pressure,etc.,with high acceptance and good compliance of patients,and the quality of life of patients can be improved.In this paper,under the premise of TCM syndrome differentiation and treatment,from the perspectives of wind,fire,phlegm,blood stasis,deficiency,etc.,taking the principles of quenching and calming wind to dredge collaterals and relieve spasm,clearing away heat and reducing fire to restore pulse function and relieve spasm,promoting blood circulation and removing blood stasis to expel phlegm and relieve spasm,and strengthening Qi(气)and tonifying to stabilize heart and relieve spasm,the etiology and pathogenesis,syndrome classification,and TCM differentiation and treatment of atrial fibrillation in recent years were summarized to provide ideas for TCM treatment of atrial fibrillation.
