摘要
目的:探讨沉默信息调节因子相关酶3/活性氧/缺氧诱导因子1α(SIRT3/ROS/HIF-1α)信号通路在盆底器官脱垂(POP)发生发展中的作用及可能机制。方法:选取2022年4月~2022年12月三明市第一医院行全子宫切除术+阴道前后壁修补+阴道断端悬吊术的POP患者20例为POP组,另选取同期因其他良性妇科疾病行全子宫切除术的20例为非POP组,分别取两组患者少许阴道顶端前壁组织,比较两组阴道前壁组织SIRT3、ROS、HIF-1α蛋白表达、平滑肌含量及成纤维细胞含量。取POP组和非POP组阴道前壁组织标本各4份,酶消化法培养原代人阴道壁成纤维细胞,分为非POP组、POP组、SIRT3 siRNA组。对比各组成纤维细胞中ROS含量、SIRT3、HIF-1α蛋白表达情况、细胞增殖情况、细胞凋亡情况。结果:患者组织标本结果:POP组ROS、HIF-1α蛋白表达水平高于非POP组,差异有统计学意义(P<0.05),SIRT3蛋白表达水平低于非POP组,差异有统计学意义(P<0.05)。POP组平滑肌含量、成纤维细胞含量少于非POP组,差异有统计学意义(P<0.05)。细胞结果表明:各组ROS含量、HIF-1α蛋白表达水平对比,非POP组<POP组<SIRT3 siRNA组,差异有统计学意义(P<0.05)。各组SIRT3蛋白表达水平对比,非POP组>POP组>SIRT3 siRNA组,差异有统计学意义(P<0.05)。各组人阴道成纤维细胞增殖对比,非POP组>POP组>SIRT3 siRNA组,差异有统计学意义(P<0.05);细胞凋亡率对比,非POP组<POP组<SIRT3 siRNA组,差异有统计学意义(P<0.05)。结论:SIRT3/ROS/HIF-1α信号通路相关分子在POP中表达异常,SIRT3低表达使ROS生成增加,HIF-1α通路异常激活,促进POP的发生发展。
Objective To investigate the role and possible mechanism of silenced information regulator related enzyme 3/reactive oxygen species/hypoxia-inducible factor 1α(SIRT3/ROS/HIF-1α)signaling pathway in the occurrence and development of pelvic floor organ prolapse(POP).Methods Twenty patients with POP who underwent surgical treatment in Sanming First Hospital from April 2022 to December 2022 were selected as the POP group,and another 20 patients who underwent surgical treatment for other benign gynecological diseases during the same period were selected as the non-POP group.The expression of SIRT3,ROS,HIF-1αprotein,smooth muscle content,and fibroblast content was compared between the two groups.The anterior vaginal wall tissue samples of the POP group and non-POP group were collected from four patients in each group.Primary human vaginal wall fibroblasts were cultured with the enzyme digestion method and divided into the non-POP group,POP group,and SIRT3 siRNA group.The ROS content,SIRT3,HIF-1αprotein expression,cell proliferation,and cell apoptosis were compared in the constituent fibrocytes.Results The expression levels of ROS and HIF-1αprotein in the POP group were higher than those in the non-POP group(P<0.05),and the expression level of SIRT protein 3 was lower than that in the non-POP group(P<0.05).The content of smooth muscle and fibroblasts in the POP group was lower than that in the non-POP group(P<0.05).The results showed as follows:The ROS content and HIF-1αprotein expression levels of all groups were compared,non-POP grouP<POP grouP<SIRT3 siRNA group(P<0.05).The expression levels of SIRT3 protein in all groups were compared:non-POP group>POP group>SIRT3 siRNA group(P<0.05).The proliferation of human vaginal fibroblasts was compared across three groups:non-POP,POP,and SIRT3 siRNA.The results showed that the proliferation rate was highest in the non-POP group,followed by the POP group,and lowest in the SIRT3 siRNA group(P<0.05).Additionally,the apoptosis rate was significantly lower in the non-POP group compared to both the POP group and the SIRT3 siRNA group(P<0.05).Conclusions The expression of molecules related to the SIRT3/ROS/HIF-1αsignaling pathway is abnormal in POP.The low expression of SIRT3 increases the production of ROS,and the abnormal activation of the HIF-1αpathway promotes the occurrence and development of POP.
