摘要
目的分析低剂量甲泼尼龙琥珀酸钠与布地奈德联合用药对重症肺炎支原体肺炎(SMPP)患儿炎症及免疫指标的影响。方法选取2022年8月至2024年7月上饶市妇幼保健院收治的SMPP患儿80例,按不同治疗方案分为两组,各40例。对照组给予布地奈德治疗,观察组在对照组基础上联合低剂量甲泼尼龙琥珀酸钠治疗。对比两组临床症状及体征、肺部感染情况、炎症指标、免疫指标、安全性。结果观察组退热时间、止咳时间、肺部阴影缩减时间、肺部啰音消失时间分别为(4.91±1.64)d、(6.75±1.40)d、(7.84±1.51)d、(3.87±0.42)d,均短于对照组的(7.13±2.18)d、(10.82±2.31)d、(10.35±2.36)d、(6.14±0.93)d;治疗后,观察组临床肺部感染评分(CPIS)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平分别为(3.31±0.61)分、(28.34±5.58)pg/mL、(2.47±0.82)mg/L,均低于对照组的(4.89±0.76)分、(38.19±6.54)pg/mL、(3.84±1.32)mg/L;观察组免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平分别为(2.51±0.67)g/L、(1.69±0.31)g/L、(8.87±1.75)g/L,均高于对照组的(2.04±0.53)g/L、(1.29±0.24)g/L、(7.45±1.23)g/L,有统计学差异(P<0.001);两组不良反应均较少,对比无统计学差异(P>0.05)。结论对SMPP患儿采取低剂量甲泼尼龙琥珀酸钠联合布地奈德治疗的效果确切,能够有效缩短症状及体征缓解时间,减轻炎症反应,提升免疫力,且安全可靠。
Objective To analyze the effect of low-dose methylprednisolone sodium succinate combined with budesonide on inflammatory and immune indexes in children with severe Mycoplasma pneumoniae pneumonia(SMPP).Methods A total of 80 children with SMPP admitted to Shangrao Maternal and Child Health Hospital from August 2022 to July 2024 were selected and divided into two groups according to different treatment schemes,with 40 cases in each group.The control group was treated with budesonide,and the observation group was treated with low-dose methylprednisolone sodium succinate on the basis of the control group.The clinical symptoms and signs,pulmonary infection,inflammatory indexes,immune indexes and safety of the two groups were compared.Results The antipyretic time,cough suppression time,reduction time of lung shadows,disappearance time of lung rales in the observation group was(4.91±1.64)d,(6.75±1.40)d,(7.84±1.51)d,and(3.87±0.42)d,re-spectively,which were shorter than those of the control group[(7.13±2.18)d,(10.82±2.31)d,(10.35±2.36)d,and(6.14±0.93)d].After treatment,the clinical pulmonary infection score(CPIS),interleukin-6(IL-6),and C-reactive pro-tein(CRP)levels in the observation group were(3.31±0.61)points,(28.34±5.58)pg/mL,and(2.47±0.82)mg/L,which were lower than those of the control group[(4.89±0.76)points,(38.19±6.54)pg/mL,and(3.84±1.32)mg/L].The lev-els of immunoglobulin A(IgA),immunoglobulin M(IgM),and immunoglobulin G(IgG)in the observation group were(2.51±0.67)g/L,(1.69±0.31)g/L,and(8.87±1.75)g/L,respectively,which were higher than those of the control group[(2.04±0.53)g/L,(1.29±0.24)g/L,and(7.45±1.23)g/L],with statistical differences(P<0.001).Both groups had fewer adverse reactions,and there was no statistically significant difference(P>0.05).Conclusion The effect of low-dose meth-ylprednisolone sodium succinate combined with budesonide in the treatment of SMPP children is exact,which can effectively shorten the relief time of symptoms and signs,reduce inflammatory reaction,improve immunity,and is safe and reliable.
