摘要
目的探讨细胞角蛋白20(CK20)、CDX2、肿瘤蛋白53(P53)在结直肠癌中的表达与临床病理特征及预后的相关性。方法选取2022年1月至2023年6月于福建中医药大学附属人民医院进行就诊的结直肠癌患者80例,检测患者癌组织中的CK20、CDX2、P53的表达情况,根据其不同的临床病理特征进行分组比较,随访6个月后,根据其预后情况将所有患者分为预后良好组和预后不良组,比较两组患者的CK20、CDX2、P53的表达情况,并采用多因素Logistic回归分析CK20、CDX2、P53与预后的关系,采用Pearson相关系数检测CK20、CDX2、P53表达与临床病理特征之间的关系。结果CK20、CDX2、P53的阳性表达水平在≥50岁、中高分化程度、浆膜浸润型、淋巴结发生转移患者体内阳性表达水平更高(P<0.05)。预后良好组患者的体内CDX2、P53阳性表达水平低于预后不良组(P<0.05)。由Logistic回归分析可得,CDX2、P53是结直肠癌预后的独立危险因素(P<0.05)。由Pearson分析可知,CK20表达与年龄、肿瘤分化程度、病理类型以及淋巴转移均呈正相关性(P<0.05)。结论CK20、CDX2、P53在结直肠癌中的表达水平在疾病的发展中具有重要意义,与临床病理特征均具有一定相关性,其中CDX2和P53与患者预后情况也具有一定相关性,可帮助临床对疾病进展进行评估。
Objective To investigate the correlation between the expression of cytokeratin 20(CK20),CDX2 and tumor protein 53(P53)in colorectal cancer and its clinicopathological features and prognosis.Methods A total of 80 patients with rectal cancer from People’s Hospital of Fujian University of Chinese Medicine from January 2022 to June 2023 were selected.The expression of CK20,CDX2 and P53 in cancer tissues of patients was detected and compared according to different clinicopathological characteristics.After 6 months of follow-up,all patients were divided into good group and bad group according to their prognosis.The expressions of CK20,CDX2 and P53 in the two groups were compared,and the relationship between CK20,CDX2 and P53 and prognosis was analyzed by multivariate Logistic regression.Pearson correlation coefficient(Pearson)was used to detect the relationship between CK20,CDX2,P53 expression and clinicopathological features.Results The positive expression levels of CK20,CDX2,and P53 were higher in patients aged≥50 years,with moderate to high differentiation,serosal invasion,and lymph node metastasis(P<0.05).The positive expression levels of CDX2 and P53 in the good prognosis group were significantly lower than those in the poor prognosis group(P<0.05).Logistic regression analysis showed that CDX2 and P53 were independent risk factors for pathological features and prognosis(P<0.05).According to Pearson,CK20 expression was positively correlated with age,tumor differentiation,pathological type and lymphatic metastasis(P<0.05).Conclusions The expression levels of CK20,CDX2 and P53 in colorectal cancer play an important role in the development of the disease and are correlated with clinical case characteristics to a certain extent.CDX2 and P53 also have a certain correlation with the prognosis of patients,which can help clinical evaluation of disease progression.
