摘要
目的探讨临床药师在自身免疫性胃炎(AIG)并多种病原体感染患儿治疗中的作用。方法回顾医院收治的1例自身免疫性胃炎并多种病原体(幽门螺杆菌、肺炎支原体、链球菌等)感染女性患儿(16岁)的治疗过程。临床药师通过查询国内外文献和药品说明书,并结合患者病史及实验室检查结果,分别于患儿入院第1天建议以阿奇霉素治疗肺炎支原体(后因出现胃肠道反应予维生素B6减轻症状),苯唑西林抗肺炎链球菌;第3天建议予艾司奥美拉唑镁+胶体果胶铋抗幽门螺杆菌;第5天建议予环磷酰胺行免疫抑制治疗;第7天建议予苄星青霉素抗感染。结果临床医师采纳临床药师建议优化治疗方案,最终患儿腹痛,恶心及呕吐等症状得到控制,体温恢复正常,于入院第8天病情好转出院。结论临床药师的参与进一步优化了用药方案,保证了患儿临床用药的安全性。
Objective To investigate the role of the clinical pharmacists in the treatment of child with autoimmune gastritis(AIG)complicated with multiple pathogen infections.Methods The treatment process of a 16-year-old female child patient with AIG and multiple pathogens(Helicobacter pylori,Mycoplasma pneumoniae,Streptococcus,etc.)infection in the hospital was reviewed.Clinical pharmacists recommended azithromycin to treat Mycoplasma pneumoniae(and then vitamin B6 was given to relieve symptoms due to gastrointestinal reaction)and oxacillin against Streptococcus pneumoniae on the 1st day after admission,esmeprazole magnesium+colloidal bismuth pectin were given to against Helicobacter pylori on the 3rd day after admission;immunosuppressive therapy with cyclophosphamide was recommended on the 5th day after admission;benzathine benzylpenicillin was given for anti-infection on the 7th day after admission.Results The clinician adopted the clinical pharmacist′s suggestions to optimize the treatment plan.Finally,the child′s symptoms of abdominal pain,nausea and vomiting were controlled,the child′s body temperature returned to normal,the condition improved and the child was discharged on the 8th day after admission.Conclusion The participation of clinical pharmacists can further optimize the medication scheme and ensure the safety of clinical medication for child.
