摘要
目的探讨临床药师在临床安全使用吸入用布地奈德混悬液中的作用。方法回顾医院收治的2例联用吸入用布地奈德混悬液、硫酸特布他林雾化吸入用溶液及吸入用乙酰半胱氨酸溶液后出现白细胞计数升高(以中性粒细胞绝对值升高为主)的老年患者的治疗经过。临床药师结合患者病情、用药史,并查询文献,认为该不良反应为布地奈德导致,且为该药新的不良反应,并经Naranjo评分(均为6分)证实两者“很可能相关”。故临床药师建议医师停用布地奈德。结果医师采纳建议。停药后患者不良反应逐渐减轻,病情稳定后出院。结论临床药师参与患者的临床用药治疗,有助于及时识别及处理药品不良反应,保障患者的用药安全。
Objective To investigate the role of clinical pharmacists in the safe use of Budesonide Suspension for Inhalation.Methods The treatment processes of two elderly patients admitted to the hospital who experienced an increase in white blood cell count(mainly absolute neutrophil count)after the combined use of Budesonide Suspension for Inhalation,Terbutaline Sulfate Nebulized Inhalation Solution,and Acetylcysteine Solution for Inhalation were reviewed.Based on the patient′s condition,medication history,and literature review,the clinical pharmacist believed that the adverse reaction was induced by Budesonide Suspension for Inhalation and was a new adverse reaction of the drug,then confirmed that the usage of Budesonide Suspension for Inhalation and the occurence of the adverse reaction were″likely related″according to the Naranjo score(both patients obtained six points).Therefore,the clinical pharmacist suggested that physicians should stop using Budesonide Suspension for Inhalation.Results The physician adopted the suggestions.After discontinuing the medication,the patient′s adverse reactions gradually reduced and the condition stabilized before being discharged.Conclusion It is helpful for the clinical pharmacists′participation in patients′clinical medication treatment to timely identify and handle adverse drug reactions,and ensure the safety of patients′medication.
