摘要
目的 设计一种新型且标准统一、可复制的体位垫,并评价其在肺小结节穿刺定位术后的应用效果。方法 选取2022年7月至2023年7月于上海交通大学医学院附属瑞金医院行肺小结节穿刺定位后取侧卧位或俯卧位的120例患者,用随机信封法分成对照组和实验组,各60例。对照组患者行穿刺定位术后采用传统体位安置法,实验组患者采用自制体位垫安置体位。对2组患者的舒适度、要求变换体位的诉求次数、局部皮肤组织的受压情况、穿刺后躁动、脱针率情况进行比较。结果 实验组患者定位术后等待手术时要求变换体位的诉求次数、俯卧位下局部皮肤组织的受压情况和穿刺后躁动情况均优于对照组(P均<0.05)。实验组患者在穿刺定位术后的舒适度各维度得分(除精神维度)及总分均高于对照组(P均<0.05)。结论 肺小结节穿刺定位后采用自行设计的新型体位垫可改善患者的舒适度,使用效果较好,且术中和术后均无不良事件的发生,患者术后康复出院。该体位垫设计科学、合理,适合于需要安置非仰卧位穿刺定位术后的患者,可按设计复制便于统一标准。
Objective To design a new type of positioning pad with unified and reproducible standards and to evaluate its application effect after puncture localization of small pulmonary nodules.Methods A total of 120 patients who underwent the puncture localization of small pulmonary nodules in a lateral or prone position at Ruijin Hospital,Shanghai Jiao Tong University School of Medicine from July 2022 to July 2023 were randomly divided into a control group and an experimental group using a random envelope method,with 60 patients in each group.The patients in the control group were placed in a traditional position after undergoing the puncture localization surgery,and the patients in the experimental group were placed in a position using self-designed positioning pads.The comfort level,the number of requests for changing positions,the degree of local skin tissue compression,the incidence of agitation after puncture,and the needle dislodgement rate of the two groups were compared.Results The number of requests for changing positions while waiting for surgery after localization,the degree of local skin tissue compression in the prone position,and the incidence of agitation after puncture in the experimental group were better than those in the control group(P<0.05).The scores of all dimensions of comfort(except the mental dimension)and total score in the experimental group after puncture localization surgery were higher than those in the control group(P<0.05).Conclusion After the puncture localization of small pulmonary nodules,the use of a new type of self-designed positioning pads can improve the patients’comfort level,and the use effect is good.There were no adverse events during or after surgery,and patients recover and are discharged after surgery.The positioning pad is scientifically and reasonably designed for patients who need to be placed in a non-supine position after puncture localization,and can be reproduced as designed to facilitate unified standards.
