摘要
目的 分析基于医护一体化的术后精细化护理在行腺样体扁桃体切除术患儿中的应用价值。方法 选取深圳市龙岗中心医院2021年1月至2023年12月收治的206例拟行腺样体扁桃体切除术患儿开展前瞻性对照研究。使用简单随机抽样法将患儿分别纳入观察组(103例)、对照组(103例),2组均接受常规术后护理,观察组加用基于医护一体化的术后精细化护理。对比2组患儿的术后恢复时间、症状消失时间、疼痛程度和并发症发生率。结果 相较于对照组,观察组的切口愈合时间更早,住院时间更短(P均<0.05)。观察组鼻翼扇动、流涕、鼻塞、睡眠呼吸粗重的消失时间均较对照组更早(均P<0.05)。观察组在术后12 h、术后24 h、术后48 h和术后72 h时的面部、腿部、活动、哭泣和可安慰性疼痛评估量表评分均较同期对照组更低(P均<0.05),且2组均低于麻醉清醒时(P均<0.05)。观察组的并发症总发生率较对照组更低(P<0.05)。结论 基于医护一体化的术后精细化护理能够促进腺样体扁桃体切除术后患儿尽早恢复、缩短症状消失时间,且有助于缓解术后疼痛,降低术后并发症发生率。
Objective To analyze the application value of postoperative refined nursing based on integrated medical-nursing in children undergoing adenotonsillectomy.Methods A prospective controlled study was conducted on 206 children undergoing adenotonsillectomy admitted to Longgang Central Hospital of Shenzhen from January 2021 to December 2023.The patients were divided into an observation group(103 cases)and a control group(103 cases)using a simple random sampling method.Both groups received routine nursing after operation,while the observation group received an additional postoperative refined nursing based on integrated medical-nursing.The postoperative recovery time,symptom disappearance time,pain degree,and the incidence of complications were compared between the two groups.Results Compared with the control group,the observation group showed earlier incision healing time and shorter length of hospital stay(P<0.05).The disappearance time of nasal flaring,runny nose,nasal obstruction,and heavy breathing during sleep in the observation group was earlier than that in the control group(P<0.05).The Face,Legs,Activity,Cry,and Consolability scale scores of the observation group at 12 h,24 h,48 h and 72 h after operation were lower than those of the control group at the same period(P<0.05).The scores in both groups were lower than those at the time of anesthesia emergence(P<0.05).The overall incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The implementation of the postoperative refined nursing based on integrated medical-nursing can promote earlier recovery and shorten the symptom disappearance time in children after adenotonsillectomy,and contribute to alleviating postoperative pain and reduce the incidence of postoperative complications.
