摘要
目的 探讨恶性血液病患者经外周静脉穿刺的中心静脉导管(PICC)置管支持性照护现状及影响因素。方法 选取2024年1~12月新疆维吾尔自治区人民医院收治的110例行PICC置管恶性血液病患者为研究对象,采用癌症患者支持性照护需求量表(SCNS-SF34)分析患者的支持性照护需求现状,采用单因素及多因素Logistic回归模型分析行PICC置管恶性血液病患者支持性照护需求的相关影响因素。结果 110例行PICC置管恶性血液病患者的SCNS-SF34总分为94.62±12.19分,患者支持性照护需求处于中等水平。多因素Logistic回归分析结果显示,年龄>60岁、文化程度为初中及以下、家庭居住地为农村、家庭人均月收入<4 000元、PICC留置时间≥6个月均是影响行PICC置管恶性血液病患者支持性照护需求的独立风险因素(P均<0.05)。结论 针对年龄>60岁、文化程度为初中及以下、家庭居住地为农村、家庭人均月收入<4 000元、PICC留置时间≥6个月的恶性血液病PICC置管患者,医护人员应为其实施针对性干预措施,以更好地满足其支持性照护需求。
Objective To explore the current status and influencing factors of supportive care for patients with malignant hematological diseases undergoing peripherally inserted central venous catheter(PICC)catheterization.Methods A total of 110 patients with malignant hematological diseases who underwent PICC catheterization at the People's Hospital of Xinjiang Uygur Autonomous Region from January to December 2024 were selected as the subjects of the study.The Supportive Care Needs Survey-Short Form 34(SCNS-SF34)for cancer patients was used to analyze the current status of the patientssupportive care needs.Univariate and multivariate Logistic regression models were used to analyze the relevant influencing factors of supportive care needs in patients with malignant hematological diseases treated with PICC catheterization.Results The total SCNS-SF34 scores of 110 patients with malignant hematological diseases undergoing PICC catheterization was 94.62±12.19 points,and the level of supportive care needs for the patients was moderate.The results of multivariate Logistic regression analysis showed that age>60 years old,the education level of junior high school or below,family residence in rural areas,family per capita monthly income<4000 yuan,and PICC retention time≥6 months were all independent risk factors affecting the supportive care needs of PICC patients with malignant hematological diseases(P<0.05).Conclusion According to the PICC catheterization patients with malignant hematological diseases who are older than 60 years old,have a junior high school education or below,live in rural areas,have a per capita monthly income of<4000 yuan,and have PICC indwelling time≥6 months,medical staff should implement targeted intervention measures to better meet their supportive care needs.
