摘要
目的 探讨多学科康复干预联合叙事心理护理在慢性心力衰竭合并衰弱患者中的应用效果。方法 选取2022年11月至2023年10月厦门大学附属心血管病医院收治的103例慢性心力衰竭合并衰弱患者,按随机数字表法分为对照组(51例)和观察组(52例)。对照组接受常规康复护理+叙事心理护理,观察组实施多学科康复干预+叙事心理护理,评估2组的护理效果。结果 随访3个月时,观察组的心功能指标(心输出量、左室射血时间、B型尿钠肽、左室射血分数、6分钟步行测试)水平均优于对照组,Tilburg衰弱量表、中文版恐惧疾病进展简化量表和明尼苏达心力衰竭生活质量量表评分均低于对照组,欧洲心力衰竭自我护理行为量表各维度评分均高于对照组(P均<0.05),且2组均优于护理前(P均<0.05)。结论 多学科康复干预联合叙事心理护理可改善慢性心力衰竭合并衰弱患者的心功能和衰弱情况,缓解其疾病恐惧进展,提高其自我管理能力和生活质量。
Objective To explore the application effect of multidisciplinary rehabilitation intervention combined with narrative psychological nursing in patients with chronic heart failure and frailty.Methods A total of 103 patients with chronic heart failure and frailty admitted to Xiamen Cardiovascular Hospital Xiamen University from November 2022 to October 2023 were randomly divided into a control group(51 cases)and an observation group(52 cases)using a random number table method.The control group received routine rehabilitation nursing and narrative psychological nursing,while the observation group received multidisciplinary rehabilitation intervention and narrative psychological nursing.The nursing effects of both groups were evaluated.Results At a 3-month follow-up,the observation group showed better levels of cardiac function indicators(cardiac output,left ventricular ejection time,brain natriuretic peptide,left ventricular ejection fraction,6-minute walking test)compared with the control group.The scores of Tilburg Frailty Indicator,the Fear of Progression Questionnaire-Short Form in Chinese version,and the Minnesota Living with Heart Failure Questionnaire were all lower in the observation group than those in the control group.The scores of European Heart Failure Self-care Behaviour Scale across all dimensions were higher in the observation group than those in the control group(P<0.05),and the scores of both groups were better than pre-nursing scores(P<0.05).Conclusion The multidisciplinary rehabilitation intervention combined with narrative psychological nursing can improve the cardiac function and frailty of patients with chronic heart failure and frailty,alleviate their fear of progression,and enhance their self-management ability and quality of life.
