摘要
全球变暖导致极端高温天气增多,热射病等热相关疾病发病率上升。热射病以高死亡率为特征,常引发全身炎症反应、弥散性血管内凝血和多脏器功能衰竭。血管内皮屏障由内皮细胞、细胞间连接和血管糖萼构成,是调节运输、凝血、氧合和通透性的关键界面。热应激可直接或间接损伤血管内皮屏障,导致功能障碍,进而引发热射病。尤其在弥散性血管内凝血和多脏器功能障碍的早期,血管内皮屏障损伤已显著存在。本综述旨在加深对血管内皮屏障及其在热射病中损伤机制的认识,为未来热射病的干预和预后评估提供思路。
Global warming has led to an increase in extreme high temperature weather and an increase in the incidence of heat-related diseases such as heat stroke(HS).Heat stroke is characterized by a high mortality rate,and often causes systemic inflammatory response,disseminated intravascular coagulation and multiple organ failure.The vascular endothelial barrier is composed of endothelial cells,intercellular junctions,and vascular glycocalyx,and is a key interface regulating transport,coagulation,oxygenation,and permeability.Heat stress can directly or indirectly damage the vascular endothelial barrier,leading to dysfunction,and then causing heat stroke.Especially in the early stages of disseminated intravascular coagulation and multiple organ dysfunction,vascular endothelial barrier damage is already significant.This review aims to enhance the understanding of the vascular endothelial barrier and its damage mechanism in heat stroke,and provide insights for future HS intervention and prognosis assessment.
