摘要
目的:分析自拟温中平胃汤联合龙砂开阖六气针法治疗腹泻型肠易激综合征(IBS-D)的临床效果。方法:选取2023年6月-2024年6月于本院接受治疗的脾虚寒湿型IBS-D患者108例,随机分为试验组(54例)和对照组(54例)。所有患者均接受常规治疗,在此基础上,试验组予以自拟温中平胃汤联合龙砂开阖六气针法治疗,均持续治疗4周。分别于治疗前后,检查所有受试者内脏敏感度(初始感觉阈值、排便阈值、疼痛阈值)、脑-肠轴功能[P物质(SP)、5-羟色胺(5-HT)、血管活性肠肽(VIP)]、Th1/Th2免疫平衡相关指标[干扰素-γ(IFN-γ)、白介素(IL)-4、IL-5],并通过中医症候积分、IBS症状严重程度积分(IBS-SSS)、IBS生活质量评分(IBS-QOL)变化进行效果评价,同时记录两组近期疗效、药物不良反应发生情况。结果:治疗前两组所有观察指标的差异无统计学意义(P>0.05)。治疗后,两组初始感觉阈值、排便阈值、疼痛阈值以及SP、IL-4、IL-5均显著升高,试验组更高于对照组(P<0.01);而两组5-HT、VIP、IFN-γ、IBS-SSS评分、IBS-QOL评分以及中医症候积分均明显降低,试验组较对照组更低(P<0.01)。试验组的总有效率高于对照组(94.44%vs 77.78%,P<0.05)。结论:自拟温中平胃汤和龙砂开阖六气针法联合治疗能够有效改善脾虚寒湿型IBS-D患者内脏高敏感性和脑-肠轴功能失调,并阻滞病灶炎症反应,进一步提高临床疗效。
Objective:To explore the clinical effect of self-made Wenzhong Pingwei decoction combined with Longsha Kaihe Six Qi acupuncture in the treatment of diarrhea predominant irritable bowel syndrome(IBS-D).Method:108 spleen deficiency cold dampness type IBS-D patients from June 2023 to June 2024 were randomly divided into an experimental group(54 cases)and a control group(54 cases).All patients received routine treatment,and on this basis,the experimental group was treated with self formulated Wenzhong Pingwei decoction combined with Longsha Kaihe Six Qi acupuncture for 4 weeks.Before and after treatment,visceral sensitivity(initial sensory threshold,defecation threshold,pain threshold),brain gut axis function[substance P(SP),serotonin(5-HT),vasoactive intestinal peptide(VIP)],Th1/Th2 immune balance related indicators[interferon-γ(IFN-γ),interleukin(IL)-4,IL-5]were examined in all subjects,and the effect was evaluated by changes in traditional Chinese medicine symptom score,IBS symptom severity score(IBS-SSS),and IBS quality of life score(IBS-QOL).At the same time,the recent efficacy and occurrence of adverse drug reactions in both groups were recorded.Results:There were no statistically significant differences in all observed indicators between the two groups before treatment(P>0.05).After treatment,both groups showed significant increases in initial sensory threshold,defecation threshold,pain threshold,as well as SP,IL-4,IL-5,and the experimental group was higher than the control group(P<0.01).However,the 5-HT,VIP,IFN-γ,IBS-SSS score,IBS-QOL score,and traditional Chinese medicine syndrome score were significantly reduced in both groups,and the experimental group had lower scores than the control group(P<0.01).In addition,the total effective rate in the experimental group was higher than that in the control group(94.44%vs.77.78%,P<0.05).Conclusion:For patients with spleen deficiency cold dampness type IBS-D,the combination treatment of self-made Wenzhong Pingwei decoction and Longsha Kaihe Six Qi acupuncture can effectively improve visceral hypersensitivity,brain gut axis dysfunction,block lesion inflammatory response,and further improve clinical efficacy.
