摘要
目的探讨涤痰活血方联合曲美他嗪对老年慢性充血性心力衰竭患者血流频谱及左心室功能的影响。方法选取60例老年慢性充血性心力衰竭患者,随机分为联合组和对照组,每组30例。对照组采用曲美他嗪治疗,联合组在对照组基础上联合涤痰活血方治疗,治疗周期均为4周。比较2组患者临床疗效、血流频谱指标、左心室功能及不良反应发生情况。结果治疗后,联合组总有效率显著高于对照组(P<0.05)。联合组二尖瓣舒张早期峰值速度(early diastolic peak velocity,E)/舒张晚期峰值速度(late diastolic peak velocity,A)、等容舒张时间(isovolumic relaxation time,IVRT)、E峰减速时间(early diastolic velocity deceleration time,EDT)、左心室射血分数(left ventricular ejection fractions,LVEF)均显著高于对照组(P<0.05),左心室收缩末期内径(left ventricular endsystolic diameter,LVESD)、左心室舒张末期内径(left ventricular enddiastolic diameter,LVEDD)均显著低于对照组(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论涤痰活血方联合曲美他嗪治疗老年慢性充血性心力衰竭临床疗效较好,可有效改善血流动力学水平,促进左心室功能恢复,且无明显不良反应。
Objective To investigate the effects of Ditan Huoxue formula combined with trimetazidine on blood flow spectrum and left ventricular function in elderly patients with chronic congestive heart failure.Methods Sixty elderly patients with chronic congestive heart failure were randomly divided into the combination group and the control group,with 30 cases in each group.The control group was treated with trimetazidine,while the combination group was treated with Ditan Huoxue formula on the basis of the control group.The treatment period was 4 weeks for both groups.The clinical efficacy,blood flow spectrum indicators,left ventricular function and incidence of adverse reactions were compared between the two groups of patients.Results After treatment,the total effective rate of the combination group was significantly higher than that of the control group(P<0.05).The combination group had significantly higher early diastolic peak velocity(E)/late diastolic peak velocity(A),isovolumic relaxation time(IVRT),early diastolic velocity deceleration time(EDT)and left ventricular ejection fraction(LVEF)than the control group(P<0.05).And the left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD)were significantly lower than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups of patients(P>0.05).Conclusion The combination of Ditan Huoxue formula and trimetazidine has good clinical efficacy in treating elderly patients with chronic congestive heart failure.It could effectively improve hemodynamic levels,promote left ventricular function recovery,and had no significant adverse reactions.
