摘要
目的探讨真实世界研究数智化平台在老年骨质疏松椎体压缩性骨折(osteoporotic vertebral compression fractures,OVCF)专病中的应用价值。方法依据临床管理及研究的需要构建包含EDC电子数据采集、ePRO电子患者报告结局、eHealth康复训练与健康宣教、eQuality数据质控、eInsight数据洞察分析5个核心模块的真实世界研究数智化平台,依托该平台建立综合健康管理路径对老年骨质疏松症患者进行健康管理,建立数据自动核查体系保证数据收集质量,同时利用数据洞察分析工具筛选出再骨折的危险因素并建立可靠的再骨折预测模型。结果构建的研究型数据库连续入组302例OVCF老年患者,采用智能信息化手段完成了部分老年骨质疏松症患者的长期管理,获得了质量较高的骨质疏松症患者数据并实行了科学研究的目的。1年内发生再骨折的住院率为24.17%(73/302),病死率为5.62%(17/302),明显改善了患者术后康复水平。结论通过真实世界研究数智化平台的构建可以实现OVCF老年患者的院后长期管理,获得高质量的真实世界数据,实现了科研与临床、治疗与康复的紧密结合。为老年OVCF治疗领域提供预后评价体系、综合管理体系提供了理论依据和数据支持。
Objective To investigate the application value of real-world research data intelligence platform in the specific disease of senile osteoporosis vertebral compression fracture.Methods According to the needs of clinical management and research,a real-world research data intelligent platform including EDC electronic data collection,ePRO electronic patient report outcome,eHealth rehabilitation training and health education,eQuality data quality control,and eInsight data insight analysis was built.Based on this platform,a comprehensive health management path was established for the health management of elderly osteoporosis patients.An automatic data verification system was established to ensure the quality of data collection,and data insight analysis tools were used to screen out the risk factors of refracture and establish a reliable prediction model for refracture.Results A total of 302 elderly patients with osteoporotic vertebral compression fractures were continuously enrolled in the research-based database constructed.The long-term management of some elderly patients with osteoporosis was completed by intelligent information means,and the high-quality data of osteoporosis patients were obtained,and the purpose of scientific research was implemented.The hospitalization rate for refracture within 1 year was 24.17%(73/302),and the mortality rate was 5.62%(17/302),which significantly improved the postoperative rehabilitation level of patients.Conclusion Long-term post-hospital management of elderly patients with osteoporotic vertebral compression fractures can be achieved through the construction of the real-world research data intelligence platform,and high-quality real-world data can be obtained,which realizes the close combination of scientific research and clinical,treatment and rehabilitation.To provide a prognostic evaluation system for the treatment of senile osteoporotic vertebral compression fractures and provide theoretical basis and data support for comprehensive management system.
