不同初筛方法对2型糖尿病患者肌少症的应用价值

摘要 目的评估6种常用肌少症初筛方法对2型糖尿病(type 2 diabetes,T2DM)患者肌少症发生风险的预测效果。方法研究对象为来自临沂市人民医院的790例T2DM患者。通过收集患者的一般资料、测量骨骼肌质量和其他人体体成分标志物,并进行问卷调查,评估了简易五项评分问卷(Strength,Assistance with walking,Rise from a chair,Climb stairs,and Falls,SARC-F)、小腿周径、肌少症评估表(SARC-F with Calf circumference,SARC-CalF)、迷你肌少症风险评估表(Mini Sarcopenia Risk Assessment,MSRA)-5、MSRA评分-7和简易物理性能测验(Short Physical Performance Battery,SPPB)对T2DM患者肌少症的预测效果。结果随机森林(重要性0.147)和多因素Logistic回归分析结果显示SARC-CalF评分对T2DM患者肌少症的诊断具有较好的预测效果(SARC-CalF评分,OR=1.24,95%CI:1.17~1.31)。ROC曲线分析显示SARC-CalF评分的预测效果较好,曲线下面积(area under curve,AUC)为0.84,敏感度为80.00%,特异度为87.12%。SARC-CalF评分联合握力和6 m步速可以进一步提高预测效果(AUC=0.94,敏感度=88.00%,特异度=88.26%)(Z=5.15,P<0.001)。结论SARC-CalF评分作为肌少症初筛方法在T2DM患者中具有较好的应用价值,并且联合握力和6 m步速可以进一步提高预测效果,这为早期筛查和干预T2DM患者肌少症提供了依据,有助于改善患者的生活质量。 Objective To evaluate the effectiveness of six commonly used sarcopenia screening methods in predicting the risk of sarcopenia occurrence in patients with type 2 diabetes(T2DM).Methods The study included 790 T2DM patients from Linyi People's Hospital.General information was collected,and measurements of skeletal muscle mass and other body composition markers were performed.Questionnaires were used to assess Strength,Assistance with walking,Rise from a chair,Climb stairs,and Falls(SARC-F)score,calf circumference,SARC-F with Calf circumference(SARC-CalF)score,Mini Sarcopenia Risk Assessment(MSRA)-5 score,MSRA-7 score,and Short Physical Performance Battery(SPPB)score for predicting sarcopenia in T2DM patients.Results The results of random forest analysis(importance=0.147)and multifactorial logistic regression indicate that the SARC-CalF score demonstrates a good predictive effect for diagnosing sarcopenia in patients with T2DM(SARC-CalF score,OR=1.24,95%CI:1.17-1.31).ROC curve analysis reveals that the predictive performance of the SARC-CalF score is satisfactory,with an area under the curve(AUC)of 0.84,sensitivity of 80.00%,and specificity of 87.12%.Furthermore,the predictive accuracy can be enhanced by combining the SARC-CalF score with handgrip strength and 6-meter walking speed(AUC=0.94,sensitivity=88.00%,specificity=88.26%)(Z=5.15,P<0.001).Conclusion The SARC-CalF score demonstrates good applicability as a primary screening tool for sarcopenia in patients with T2DM.When combined with grip strength and 6-meter walking speed,its predictive performance is further enhanced.These findings provide a valuable basis for early screening and intervention for sarcopenia in T2DM patients,contributing to improved quality of life.
