摘要
目的:探讨气滞血瘀型腰椎间盘突出症(LDH)采用针刺配合腹部推拿治疗的临床效果。方法:研究对象选取2021年11月—2024年3月于南昌市按摩医院门诊及住院收治的气滞血瘀型LDH患者,共计62例,通过随机数字表法分为治疗组(n=31)与对照组(n=31)。两组均采取常规腰腿部推拿治疗,在此基础上对照组予以针刺治疗,治疗组予以针刺配合腹部推拿治疗,两组均治疗2个疗程,比较两组临床疗效、腰椎功能[日本骨科协会(JOA)]、疼痛程度(VAS)、不良反应及治疗结束后3个月内复发率。结果:治疗组总有效率为93.5%,高于对照组的77.4%(P<0.05);较治疗前,两组治疗后VAS评分降低,且治疗组低于对照组,两组治疗后JOA评分提高,且治疗组高于对照组(P<0.05);两组治疗期间均未发生不良反应发生;治疗结束后3个月内,治疗组随访复发率低于对照组(P<0.05)。结论:针刺配合腹部推拿治疗气滞血瘀型LDH患者效果显著,改善腰椎功能,减轻疼痛程度,且复发率较低。
Objective:To explore the clinical effect of acupuncture combined with abdominal massage in the treatment of lumbar disc herniation(LDH)of Qi stagnation and blood stasis type.Method:A total of 62 patients with LDH of Qi stagnation and blood stasis type admitted to the Outpatient Department and Inpatient Department of Nanchang Massage Hospital from November 2021 to March 2024 were selected as the research subjects,they were randomly divided into the treatment group(31 patients)and the control group(31 patients)by the random number table method.Both groups were treated with routine lumbar and leg massage,on this basis,the control group was treated with acupuncture,and the treatment group was treated with acupuncture combined with abdominal massage,both groups were treated for 2 courses.The clinical efficacy,lumbar function[Japanese orthopaedic association(JOA)],pain degree(VAS),adverse reactions and recurrence rate within 3 months after treatment were compared between two groups.Result:The total effective rate of the treatment group was 93.5%,which was higher than 77.4%of the control group(P<0.05);compared with those before treatment,the VAS scores of two groups were decreased after treatment,and that in the treatment group was lower than that in the control group,the JOA scores of two groups were increased after treatment,and that in the treatment group was higher than that in the control group(P<0.05);no adverse reactions occurred in two groups during the treatment period;within 3 months after the end of treatment,the recurrence rate of the treatment group was lower than that of the control group(P<0.05).Conclusion:Acupuncture combined with abdominal massage has a significant effect in the treatment of patients with LDH of Qi stagnation and blood stasis type,can improve lumbar function,reduce pain,and has a low recurrence rate.
