摘要
目的:探讨在治疗血瘀证创伤性四肢骨折患者中应用改良桃红四物汤联合切开复位内固定术的效果。方法:选择中国人民解放军西部战区空军医院收治的血瘀证创伤性四肢骨折患者75例,纳入时间为2022年6月—2024年6月,按照随机数字表法分为两组,对照组37例给予切开复位内固定术治疗,观察组38例增加改良桃红四物汤治疗。观察两组临床疗效、炎症和凝血标志物、血液流变学指标、并发症。结果:与对照组(78.38%)比较,观察组总有效率(94.74%)明显更高,差异有统计学意义(P<0.05)。与对照组比较,观察组治疗后的前列环素I2(PGI2)水平明显更高,血栓素B2(TXB2)、TXB2/PGI2水平更低,差异有统计学意义(P<0.05)。与对照组比,观察组治疗后的血浆黏度(PV)、全血高切黏度(HBV)、全血低切黏度(LBV)、FIB水平明显更低,差异有统计学意义(P<0.05)。观察组并发症发生率(13.16%)显著低于对照组(35.14%),差异有统计学意义(P<0.05)。结论:对血瘀证创伤性四肢骨折患者应用改良桃红四物汤联合切开复位内固定术的疗效较为显著,能够改善血液流变学,减轻炎症反应及高凝状态,且安全性良好。
Objective:To explore the effect of Modified Taohong Siwu Decoction combined with open reduction and internal fixation surgery in treating patients with traumatic limb fractures with blood stasis syndrome.Method:Seventy-five traumatic limb fractures with blood stasis syndrome admitted to Air Force Hospital of Western Theater Command of the People's Liberation Army from June 2022 to June 2024 were selected and included.According to the random number table method,they were divided into two groups,the control group of 37 patients who received open reduction and internal fixation surgery,and the observation group of 38 patients who additional received Modified Taohong Siwu Decoction treatment.The clinical efficacy,inflammation and coagulation markers,hemorheological indicators,and complications of the two groups were observed.Result:Compared with the control group(78.38%),the total effective rate of the observation group(94.74%)was significantly higher,and the difference was statistically significant(P<0.05).Compared with the control group,the observation group showed significantly higher level of PGI2 and lower levels of TXB2 and TXB2/PGI2 after treatment,with statistical significance(P<0.05).Compared with the control group,the observation group showed significantly lower levels of plasma viscosity(PV),whole blood high shear viscosity(HBV),whole blood low shear viscosity(LBV),and FIB after treatment,with statistical significance(P<0.05).The incidence of complications in the observation group(13.16%)was significantly lower than that in the control group(35.14%),and the difference was statistically significant(P<0.05).Conclusion:The Modified Taohong Siwu Decoction combined with open reduction and internal fixation surgery has shown remarkable efficacy for traumatic limb fracture patients with blood stasis syndrome.It can improve hemorheology,reduce inflammation and hypercoagulability,and has a good safety profile.
