摘要
目的:探讨神经肌肉电刺激联合功能锻炼对重型颅脑外伤患者康复的影响。方法:选择2022年4月—2023年10月于东台市人民医院治疗的100例重型颅脑外伤患者,通过随机抽签法分为对照组(n=50)和观察组(n=50)。两组均开展机械通气治疗,其中对照组给予常规护理干预和神经肌肉电刺激,观察组则在对照组的基础上联合功能锻炼。对两组机械通气时间、ICU住院时间、住院费用和入住ICU 3、14 d后感染指标(WBC、CRP、PCT)、格拉斯哥昏迷量表(GCS)评分、NIHSS评分及并发症发生率进行比较。结果:相较于对照组,观察组机械通气时间、ICU住院时间较短,住院费用较少(P<0.05);相较于对照组,入住ICU 14 d后观察组WBC、CRP、PCT水平和NIHSS评分更低,GCS评分更高(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:将神经肌肉电刺激联合功能锻炼应用于重型颅脑外伤患者的护理干预当中,能有效缩短患者机械通气时间、住院时间,促进患者神经功能的恢复,同时有利于降低呼吸机相关性肺炎等并发症发生风险。
Objective:To explore the impact of neuromuscular electrical stimulation combined with functional exercises on the rehabilitation of patients with severe traumatic brain injury.Method:A total of 100 patients with severe craniocerebral trauma who were treated in Dongtai People's Hospital from April 2022 to October 2023 were selected and divided into the control group(n=50)and the observation group(n=50)by random lottery.Both groups received mechanical ventilation treatment.Among them,the control group was given conventional nursing intervention and neuromuscular electrical stimulation,while the observation group was combined with functional exercises on the basis of the control group.The mechanical ventilation time,ICU stay time,hospitalization cost,infection indicators(WBC,CRP,PCT),Glasgow coma scale(GCS)score,NIHSS score at 3 and 14 days after ICU admission,and the incidence of complications were compared between the two groups.Result:Compared with the control group,the mechanical ventilation time and ICU stay time in the observation group were shorter,and the hospitalization cost was lower(P<0.05).Compared with the control group,14 days after admission to the ICU,the levels of WBC,CRP,PCT and NIHSS score in the observation group were lower,and the GCS score was higher(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The application of neuromuscular electrical stimulation combined with functional exercise in the nursing intervention of patients with severe craniocerebral trauma can effectively shorten the mechanical ventilation time and hospital stay of patients,promote the recovery of neurological function of patients,and at the same time be conducive to reducing the risk of adverse reactions such as ventilator-associated pneumonia.
