摘要
目的:探讨直接前入路与后外侧入路人工全髋关节置换术(THA)的应用效果。方法:选取景德镇市第二人民医院2021年1月—2024年3月收治的择期行THA的患者80例,按随机数字表法分为直接前入路组(n=40)、后外侧入路组(n=40)。直接前入路组经直接前入路手术,后外侧入路组经后外侧入路手术。对比两组围手术期相关指标、疼痛程度、髋关节功能及生活质量。结果:相较后外侧入路组,直接前入路组术中出血量少,首次下床活动时间早,总住院时间短(P<0.05);两组手术用时对比,差异无统计学意义(P>0.05)。直接前入路组术后6、24 h数字分级评分法(NRS)评分较后外侧入路组低(P<0.05)。直接前入路组术后3、6个月Harris髋关节评分(HHS)较后外侧入路组高(P<0.05)。直接前入路组术后6个月健康调查简表(SF-36)各维度评分较后外侧入路组高(P<0.05)。结论:相较后外侧入路,直接前入路行THA可缩短康复进程,减轻术后疼痛程度,促进髋关节功能恢复,提高生活质量。
Objective:To investigate the application effect of direct anterior approach and posterolateral approach for total hip arthroplasty(THA).Method:A total of 80 patients who underwent elective THA in the Second People's Hospital of Jingdezhen from January 2021 to March 2024 were selected and divided into direct anterior approach group(n=40)and posterolateral approach group(n=40)according to random number table method.The direct anterior approach group underwent surgery through direct anterior approach and the posterolateral approach group underwent surgery through posterolateral approach.Perioperative related indexes,pain degree,hip function and quality of life were compared between the two groups.Result:Compared with the posterolateral approach group,the intraoperative blood loss in the direct anterior approach group was less,the time of first getting out of bed was earlier,and the total hospital stay was shorter(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The numerical rating scale(NRS)scores of the direct anterior approach group 6 and 24 hours after surgery were lower than those of the posterolateral approach group(P<0.05).The Harris hip scores(HHS)3 and 6 months after surgery in the direct anterior approach group were higher than those in the posterolateral approach group(P<0.05).Six months after surgery,the scores of each dimension of SF-36 in the direct anterior approach group were higher than those in the posterolateral approach group(P<0.05).Conclusion:Compared with the posterolateral approach,the direct anterior approach for THA can shorten the rehabilitation process,reduce postoperative pain degree,promote the recovery of hip function and improve the quality of life.
