摘要
目的探讨急性支气管炎患儿血液白细胞计数(WBC)、C反应蛋白(CRP)、α1-酸性糖蛋白(AAG)水平及其对抗菌治疗的指导价值。方法选取2023年4月至6月于汉中市中心医院儿科病区住院治疗的98例1~7岁急性支气管炎患儿为病例组,根据入院是否进行抗菌治疗将其分为抗菌治疗组(n=45)与非抗菌治疗组(n=53)。另选取同期于儿童保健门诊体检的74名1~7岁健康体检儿童作为对照组。比较病例组与对照组以及抗菌治疗组与非抗菌治疗组的血常规及生化指标。绘制受试者工作特征(ROC)曲线评价WBC、CRP、AAG单独及联合检测指导急性支气管炎患儿抗菌治疗的价值。结果病例组的WBC、中性粒细胞百分比(NEUT%)、CRP及AAG水平均高于对照组,差异具有统计学意义(P<0.05)。抗菌治疗组的WBC、CRP及AAG水平高于非抗菌治疗组,差异具有统计学意义(P<0.05)。ROC曲线结果显示,WBC、CRP、AAG单独及联合检测指导急性支气管炎患儿抗菌治疗的曲线下面积(AUC)分别为0.644、0.744、0.762及0.799。结论急性支气管炎患儿血液中的WBC、CRP和AAG水平会异常升高,三者联合检测可作为指导抗菌治疗决策的预测指标。
Objective To investigate the levels of white blood cell count(WBC),C-reactive protein(CRP)and α1-acid glycoprotein(AAG)in blood of children with acute bronchitis and their guiding value for antimicrobial treatment.Methods A total of 98 children with acute bronchitis aged 1-7 years who were hospitalized in the pediatric ward of Hanzhong Central Hospital from April to June 2023 were selected as case group.The children were divided into antibacterial treatment group(n=45)and non-antibacterial treatment group(n=53)according to whether they received antibacterial treatment or not.In addition,74 healthy children aged 1-7 years who underwent physical examination in the children's health clinic during the same period were selected as control group.The blood routine and biochemical indexes of the case group and the control group,as well as the antibacterial treatment group and the non-antibacterial treatment group were compared.The receiver operating characteristic(ROC)curve was drawn to evaluate the value of single and combined detection of WBC,CRP and AAG in guiding the antibacterial treatment of children with acute bronchitis.Results The WBC,neutrophilic granulocyte percentage(NEUT%),CRP and AAG levels in the case group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of WBC,CRP and AAG in the antibacterial treatment group were higher than those in the non-antibacterial treatment group,and the differences were statistically significant(P<0.05).The results of ROC curve showed that the area under curve(AUC)of WBC,CRP,AAG alone and combined detection in guiding the antibacterial treatment of children with acute bronchitis were 0.644,0.744,0.762 and 0.799,respectively.Conclusion The levels of WBC,CRP and AAG in the blood of children with acute bronchitis will increase abnormally,and the combination detection of the three can be used as a predictor to guide the decision-making of antibacterial treatment.
