摘要
目的基于NF-κB通路,探讨香砂六君子汤联合电针对术后早期炎性肠梗阻(early postoperative inflammatory small bowel obstruction,EPISBO)小鼠胃肠功能及Cajal间质细胞(interstitial cellsof Cajal,ICC)的影响。方法将40只小鼠随机分为对照组、模型组、中药组(香砂六君子汤)、电针组和中药加电针组,每组8只。除了对照组以外,其余各组小鼠采用肠管表面擦拭法建立小鼠EPISBO模型。建造模型结束后隔一天进行处理,中药组用香砂六君子汤灌肠,电针组使用电针刺激;中药加电针组使用电针刺激,并灌胃服用香砂六君子汤治疗。每日1次,连续1周。取小鼠腹主动脉血清及小肠组织;HE染色检测小鼠小肠组织的结构和病理学变化,测量小肠推进率;ELISA检测小鼠血清白细胞介素-6(interleukin-6,IL-6)与肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的浓度;Western blot检测小肠组织中c-kit(胃肠道Cajal间质细胞的特异性受体)和NF-κB通路中的关键蛋白p-p65和抑制蛋白IkB-α的蛋白表达。结果与对照组比,模型组小鼠的小肠推进率降低,小肠组织炎症程度加重,c-kit和IkB-α蛋白水平降低(P<0.001),IL-6和TNF-α水平及p-p65蛋白表达升高(P<0.001)。与模型组比较,中药组(香砂六君子汤)、电针组和中药加电针组的小肠推进率升高,小肠组织炎症程度减弱,c-kit和IkB-α蛋白水平升高(P<0.001),IL-6和TNF-α水平及p-p65蛋白表达降低(P<0.001)。中药加电针联合治疗的效果高于两者单独治疗的水平。结论香砂六君子汤联合电针可以减轻小鼠的炎症和肠梗阻的症状,并改善小鼠肠道功能,其机制可能与ICC的激活和NF-κB信号通路有关。
Objective Based on NF-κB pathway,this study explored the effect of Xiangsha Liujunzi Decoction(香砂六君子汤)combined with electroacupuncture(EA)on gastrointestinal function and interstitial cells of Cajal(ICC)of mice with early postoperative inflammatory intestinal bowel obstruction(EPISBO).Methods Forty mice(6 weeks old)were randomly allocated into control group,model group,Chinese medicine group,EA group and Chinese medicine combined with EA group,8 mice in each group.Except for the control group,all mice of the other groups were used to establish EPISBO models by wiping the intestinal surface.After the establishment of the model,the mice were treated every other day.The Chinese medicine group was given Xiangsha Liujunzi Decoction by enema,and EA group was given EA stimulation.The Chinese medicine combined with EA group was given EA stimulation and Xiangsha Liujunzi Decoction by gastric perfusion once a day for one week.The abdominal aorta serum and small intestine tissue of mice were taken out.HE staining was employed to assess the histological structure and histopathologic changes of intestinum tenue.ELISA was used to detect the concentrations of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in serum.Western blot was used to detect the protein expression of c-kit(a specific receptor for Cajal interstitial cells in the gastrointestinal tract),key protein p-p65 in the NF-κB pathway and inhibitory protein IkB-αin small intestine tissue.Results Compared with those of the control group,the small intestine propulsion rate of the model group decreased,the degree of inflammation in the small intestine tissue worsened,and the levels of c-kit and IkB-αproteins decreased(P<0.001),the levels of IL-6 and TNF-αand p-p65 protein expression increased.Compared with the model group,the Chinese medicine group(Xiangsha Liujunzi Decoction),EA group and Chinese medicine combined with EA group showed an increase in small intestine propulsion rate,a decrease in inflammation degree of small intestine tissue and an increase in the levels of c-kit and IkB-αproteins(P<0.001).The levels of IL-6 and TNF-αas well as the expression of p-p65 protein were reduced(P<0.001).The combined treatment of Chinese medicine and EA is more effective than the treatment of both alone.Conclusion Xiangsha Liujunzi Decoction combined with EA can alleviate inflammation and intestinal obstruction symptoms and improve intestinal function in mice,and its mechanism may be related to activating ICC and NF-κB signal pathway.
