摘要
目的评估中药脐部贴敷联合脊柱推拿对腹泻患儿胃肠功能恢复及促炎因子的影响。方法选取2022年2月-2023年8月于该院接受中西医诊疗方案的腹泻患儿86例,中医辨为“脾虚湿盛证”,进入对照试验。调查患儿资料,按照等比例随机编码的方式,分为对照组和观察组,各43例。对照组采用蒙脱石散治疗;在此基础上,观察组予以中药脐部贴敷联合脊柱推拿。所有患者以5d为1个疗程。系统评价疗效,记录胃肠功能恢复时间,治疗前后测定促炎因子[肿瘤坏死因子α(tumornecrosis factorα,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)]。结果对照组中总有效率81.40%(35/43)低于观察组95.35%(41/43),差异有统计学意义(P<0.05)。对照组患儿腹痛消失时间、腹泻停止时间和大便恢复时间,高于观察组,差异有统计学意义(P<0.05)。与对照组比,治疗后观察组患儿血清TNF-α、IL-6和CRP较低,差异有统计学意义(P<0.05)。结论中药脐部贴敷联合脊柱推拿可以促进胃肠功能恢复,并且抑制促炎因子的分泌,从而提高疗效。
Objective To evaluate the effects of traditional Chinese medicine umbilical cord application combined with spinal Tuina on gastrointestinal function recovery and pro-inflammatory factors in children with diarrhea.Methods From February 2022 to August 2023,86 cases of children with diarrhea who received traditional Chinese and Western medicine treatment in the hospital were selected and identified as“spleen deficiency and dampness excess syndrome”by traditional Chinese medicine.The data of the children was investigated and divided into control group and observation group with 43 cases in each group according to the method of equal proportion random coding.The control group was treated with montmorillonite powder.On this basis,the observation group was given traditional Chinese medicine umbilical application combined with spinal Tuina.All patients were treated with a course of five day.The therapeutic effect was systematically evaluated,the recovery time of gastrointestinal function was recorded,and pro-inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP)]were measured before and after treatment.Results The total effective rate(81.40%,35/43)in the control group was lower than that(95.35%,41/43)in the observation group,and the difference was statistically significant(P<0.05).The time of abdominal pain disappearance,diarrhea stop time and stool recovery time in the control group were higher than those in the observation group,and the difference was statistically significant(P<0.05).Compared with those in the control group,the serum levels of TNF-α,IL-6 and CRP in the observation group were lower after treatment,and the difference was statistically significant(P<0.05).Conclusion Traditional Chinese medicine umbilical cord application combined with spinal Tuina can promote the recovery of gastrointestinal function and inhibit the secretion of pro-inflammatory factors,so as to improve the curative effect.
