摘要
目的探讨中医方案(知葛通脉颗粒结合中药定向透药)治疗气虚血瘀型糖尿病远端对称性多发性神经病变效果。方法纳入该院收治的60例糖尿病远端对称性多发性神经病变患者(气虚血瘀型)(2022年10月—2023年7月)以随机数字表法平均分为两组,分别为常规治疗的参考组,常规治疗结合知葛通脉颗粒、中药定向透药治疗的观察组,数据观察:治疗效果、多伦多评分变化、中医证候总积分变化、血清相关活性因子变化与神经传导速度指标变化、不良反应。结果观察组的总有效率(93.33%,28/30)比参考组(73.33%,22/30)更高,P<0.05;治疗前各组患者中医证候总积分、多伦多评分、腓浅神经的感觉神经传导速度(sensory nerve conduction velocity,SCV)以及腓总神经的运动神经传导速度(motor nerve conduction velocity,MCV)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)及神经生长因子(nerve growth factor,NGF)等指标比较,P>0.05,治疗后各组患者中医证候总积分、多伦多评分、MCV、SCV、VEGF、IGF-1、NGF等指标均改善,观察组患者治疗后中医证候总积分、多伦多评分、MCV、SCV、VEGF、IGF-1、NGF等指标均优于参考组,P<0.05;观察组与参考组患者不良反应率均较低,P>0.05。结论知葛通脉颗粒与中药定向透药结合治疗糖尿病远端对称性多发性神经病变(气虚血瘀型)效果较好,且治疗安全。
Objective To investigate the effect of traditional Chinese medicine[Zhige Tongmai Granule(知葛通脉颗粒)combined with traditional Chinese medicine directional transdrug]on the treatment of Qi deficiency and blood stasis type of diabetes mellitus with distal symmetric multiple neuropathy.Methods Sixty patients with remote symmetric multiple neuropathy of diabetes mellitus(Qi deficiency and blood stasis type)admitted to the hospital(October 2022-July 2023)were divided into two groups by random number table method,which were respectively the reference group(conventional treatment)and the observation group(conventional treatment combined with Zhige Tongmai Granules and traditional Chinese medicine directed transdrug treatment).The treatment effect,Toronto score change,TCM syndrome total score change,serum related active factor change and nerve conduction velocity index change and adverse reactions of two groups were observed and compared.Results The total effective rate of the observation group(93.33%,28/30)was higher than that of reference group(73.33%,22/30)(P<0.05).Before treatment,the total TCM syndrome score,Toronto score,sensory nerve conduction velocity(SCV)of superficial peroneal nerve,motor nerve conduction velocity(MCV)of common peroneal nerve,vascular endothelial growth factor(VEGF),insulin-like growth factor-1(IGF-1)and nerve growth factor(NGF)of all groups were not significant(P>0.05).After treatment,the total TCM syndrome score,Toronto score,the values of MCV,SCV,VEGF,IGF-1,NGF and other indexes were improved in both groups,and the total TCM syndrome score,Toronto score,MCV,SCV,VEGF,IGF-1,NGF and other indexes in the observation group were better than those in the reference group(P<0.05).The adverse reaction rate of the observation group and reference group was lower(P>0.05).Conclusion Zhige Tongmai Granules combined with traditional Chinese medicine directional Tongyao in the treatment of diabetic distal symmetric multiple neuropathy(Qi deficiency and blood stasis type)is effective and safe.
