摘要
目的探讨血栓弹力图、血脂指标对2型糖尿病患者伴发大血管并发症的预测价值。方法回顾性选取2023年1月—2024年10月泰安市中医医院收治的135例2型糖尿病患者的临床资料,根据是否伴发大血管并发症将患者分为未发生组41例和发生组94例。采用单因素及多因素Logistic回归分析2型糖尿病患者伴发大血管并发症的独立危险因素。并绘制受试者工作特征(receiver operating characteristic,ROC)曲线评估血栓弹力图参数、血脂指标对2型糖尿病患者伴发大血管并发症的预测价值。结果两组患者年龄、总胆固醇(total Cholesterol,TC)、甘油三酯(triglycerides,TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、反应时间(R值)、凝血时间(K值)、凝固角(Angle)和综合凝血指数(CI值)对比,差异均有统计学意义(P均<0.05)。多因素Logistic回归分析,TG、CI值、年龄是2型糖尿病患者伴发大血管并发症的独立危险因素(OR=21.183,1.973,1.521;P均<0.05)。经ROC曲线分析,CI值的AUC为0.711(0.613~0.811)、TG水平的AUC为0.866(0.798~0.943)。结论2型糖尿病患者血栓弹力图中的CI值以及血脂指标中的TG水平升高,可提示有伴发大血管并发症的风险,具有一定的预测价值。
Objective To explore the predictive value of thrombelastogram and blood lipid index for macrovascular complications in patients with type 2 diabetes mellitus(T2DM).Methods The clinical data of 135 patients with type 2 diabetes admitted to Tai′an Hospital of Traditional Chinese Medicine from January 2023 to October 2024 were retro-spectively selected.The patients were divided into 41 cases in the non-occurrence group and 94 cases in the occur-rence group according to whether they were accompanied by macrovascular complications.Univariate and multivariate Logistic regression analysis were used to analyze the independent risk factors of macrovascular complications in pa-tients with type 2 diabetes.The receiver operating characteristic(ROC)curve was drawn to evaluate the predictive value of thrombelastogram parameters and blood lipid indexes for macrovascular complications in patients with type 2 diabetes.Results The age,total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),re-action time(R value),coagulation time(K value),coagulation angle(Angle)and comprehensive coagulation index(CI value)between the two groups,the differences were statistically significant(all P<0.05).Multivariate Logistic regres-sion analysis showed that TG,CI value and age were independent risk factors for macrovascular complications in pa-tients with type 2 diabetes(OR=21.183,1.973,1.521;all P<0.05).ROC curve analysis showed that the AUC of CI value was 0.711(0.613-0.811),and the AUC of TG level was 0.866(0.798-0.943).Conclusion The increase of CI value in thrombelastogram and TG level in blood lipid index in patients with type 2 diabetes mellitus may indicate the risk of macrovascular complications and have certain predictive value.
