摘要
IgA肾病是常见的原发性肾小球疾病,病程长且预后不良,危害甚大。中医基本病机为本虚标实,气虚最为多见,“瘀”贯穿疾病全过程。陈帮明主任医师从虚、瘀两方面展开辨证,同时结合患者体质、微观病理、检验结果,从整体到微观辨证IgA肾病。论治时将IgA肾病分为急性期与缓解期,通过补阳还五汤加减治疗缓解期的气虚,将活血药联合虫类药治疗病程中的瘀象,并根据患者检验结果加减用药,中西结合治疗IgA肾病,取得满意疗效。
IgA nephropathy(IgAN)is a common primary glomerular disorder characterized by a prolonged course and poor prognosis,exerting significant harm.The fundamental pathogenesis in traditional Chinese medicine for this disease is deficiency in the root and excess in the manifestation,with Qi deficiency being the most prevalent.The“Blood stasis”persists throughout the entire disease process.Director Physician Chen Bangming delineates the condition through the dual lenses of deficiency and stasis,concurrently taking into account constitutional factors,microscopic pathology,and examination results to establish a comprehensive diagnosis of IgAN at both macroscopic and microcosmic levels.In therapeutic approach,IgAN is categorized into acute and remission phases.The Qi deficiency in the remission stage is treated with modifications of the Buyang Huanwu Decoction.The blood stasis during the disease course is addressed by combining blood-activating drugs with insect drugs and and the medication is adjusted based on the patient’s test results.The combination of traditional Chinese and Western medicine is employed to treat IgAN to achieve satisfactory therapeutic efficacy.
