摘要
杜义斌教授从事中医药辨证论治外感类疾病已有数十年,发现外感发病多遵循“始伤于风寒或风寒湿等阴性邪气,后又迅速合于内伤郁火,从而由寒转热,并由浅入深、气病及血”的规律,据此将伤寒六经辨证体系的三阳经证治思路与温病卫气营血及三焦辨证体系结合为“气血三阳合治法”,临证以“三阳传变、由经及腑”规律随证选用相应主方,同时依据“既病防变、未病先防及正邪关系”等理念明辨正邪、病位、病性等主次关系,继而合方以经腑或脏腑同治;又在整体观念指导下按“由寒转热、由气及血”的规律总揽全局,随证再取营、血分证相应主方以气血同治,组方环环相扣、互为佐助、构思精妙。长期临床实践表明,采用该思路论治外感热病,辨证精准、疗效卓著,值得推广应用。
Professor Du Yibin has been engaged in the traditional Chinese medicine syndrome differentiation and treatment of exogenous diseases for decades.He has found that the onset of exogenous diseases often follows the law of“starting with negative pathogenic Qi such as wind cold or wind cold dampness,and then quickly merging with internal injury stagnation fire,thus turning from cold to heat,and gradually progressing from shallow to deep,Qi disease and blood”.Based on this,the three Yang meridian syndrome differentiation and treatment ideas of the six meridians of typhoid fever are combined with the warm disease,wei Qi,Ying blood and Three Jiao syndrome differentiation systems to form the“Qi blood three Yang combined treatment method”.The corresponding main formulas are selected according to the pattern of“three yang transmission and transformation,from meridians and organs”,and the concepts of“prevention and transformation of existing diseases,prevention before disease,and the relationship between positive and evil”are used to distinguish between right and wrong.The primary and secondary relationships of pathogenic factors,disease location,and disease nature are then combined to treat the meridians or organs together;Guided by the overall concept,we follow the law of“from cold to heat,from Qi to blood”to comprehensively grasp the whole situation.Based on the syndrome,we then select the corresponding main formulas for the differentiation of Qi and blood,and use Qi and blood to treat the syndrome together.The formulas are interrelated,mutually supportive,and ingeniously conceived.Long term clinical practice has shown that using this approach to treat exogenous febrile diseases has precise syndrome differentiation and remarkable therapeutic effects,and is worthy of promotion and application.
